子宫内膜癌患者癌症复发恐惧变化轨迹及因素识别:基于不同时间点的纵向研究

摘　　要：目的 探究子宫内膜癌患者癌症复发恐惧变化轨迹及影响因素。方法 选取2018年6月至2023年2月南阳医学高等专科学校第二附属医院120例子宫内膜癌患者,采用恐惧疾病进展简化量表分别于不同时间点评估患者癌症复发恐惧变化轨迹。利用增长混合模型识别轨迹类别,logistic分析识别轨迹类别的影响因素。结果 本研究识别出3条癌症复发恐惧的不同轨迹,分别为持续高水平组(12.71%)、缓慢下降组(61.86%)、低水平组(25.42%);化疗史、疾病不确定感、焦虑情绪、抑郁情绪是子宫内膜癌患者癌症复发恐惧轨迹的危险因素,希望水平、社会支持是其保护因素。结论 子宫内膜癌患者患病期间癌症复发恐惧具有一定差异,初步识别出3种不同变化轨迹,且化疗史、疾病不确定感、焦虑情绪、抑郁情绪、希望水平、社会支持是其影响因素,临床可以此为依据及时采取干预措施,以降低癌症复发恐惧对患者身心健康的不良影响。Objective To explore the change trajectory and influencing factors of cancer recurrence fear in patients with endometrial cancer.Methods A total of 120 patients with endometrial cancer from June 2018 to February 2023 in the Second Affiliated Hospital of Nanyang Medical College were selected.The fear of disease progression simplified scale was used to assess the change trajectory of cancer recurrence fear at different time points.The growth mixture model was used to identify trajectory categories,and logistic analysis was used to identify the influencing factors of trajectory categories.Results This study identified three different trajectories of cancer recurrence fear,namely,a sustained high level group(12.71%),a slowly declining group(61.86%),and a low level group(25.42%).Chemotherapy history,uncertainty about the disease,anxiety,and depression were risk factors for the trajectory of cancer recurrence fear in endometrial cancer patients,while hope level and social support were protective factors.Conclusion There are some differences in the fear of cancer recurrence in patients with endometrial cancer during the disease.Three different trajectrajectory have been preliminarily identified,and chemotherapy history,disease uncertainty,anxiety,depression,hope level,sleep quality and social support are the influencing factors.Clinical intervention measures can be taken on this basis in time.In order to reduce the negative impact of the fear of cancer recurrence on the physical and mental health of patients.
