摘  要:目的 探究计算机辅助设计/计算机辅助制作(CAD/CAM)氧化锆全瓷冠、金属烤瓷冠对牙冠修复的效果。方法 回顾性选取平顶山市第二人民医院2021年9月至2023年9月收治的85例牙冠修复患者的病历资料,根据修复方案分为两组(对照组42例,研究组43例)。对照组使用钴铬合金烤瓷冠,研究组使用CAD/CAM氧化锆全瓷冠。对比两组修复效果、牙周指标[探诊深度(PD)、龈沟出血指数(SBI)、菌斑指数(PLI)]、龈沟液中基质金属蛋白酶(MMPs)[基质金属蛋白酶-2(MMP-2)、MMP-8、MMP-9]水平及患者修复满意度。结果 研究组颜色与邻牙匹配合格率较对照组高(P<0.05);两组外形合格率均为100%,两组边缘着色及完整性合格率相比,差异无统计学意义(P>0.05);与修复前对比,修复2周、6个月后,两组PD、SBI、PLI、MMP-2、MMP-8、MMP-9水平均升高,且研究组低于对照组(P<0.05);修复3个月后,两组PD、SBI、PLI、MMP-2、MMP-8、MMP-9对比,差异无统计学意义(P>0.05);研究组修复满意度优于对照组(P<0.05)。结论 CAD/CAM氧化锆全瓷冠修复牙冠效果较好,对牙周组织刺激较小,患者满意度高。Objective To investigate the effects of computer-aided design/computer-aided manufacturing(CAD/CAM)zirconia porcelain crown and metal porcelain crown on dental crown restoration.Methods The medical records of 85 patients with crown restoration in Pingdingshan the Second People’s Hospital from September 2021 to September 2023 were retrospectively selected and divided into two groups(42 cases in control group and 43 cases in study group)according to restoration plans.The control group accepted cobalt-chromium alloy porcelain crown,and the study group accepted CAD/CAM zirconia porcelain crown.The repair effect,periodontal indexes[probing depth(PD),sulcus bleeding index(SBI),plaque index(PLI)],matrix metalloproteinases(MMPs)[matrix metalloproteinase-2(MMP-2),MMP-8,MMP-9]in gingival crevicular fluid and repair satisfaction of patients were compared between the two groups.Results The qualified rate of color matching with adjacent teeth in the study group was higher than that in the control group(P<0.05).The qualified rate of shape of two groups were 100%.There were no statistical differences in the qualified rate of edge coloring and integrity between the two groups(P>0.05).Compared with before repair,after 2 weeks and 6 months,the levels of PD,SBI,PLI,MMP-2,MMP-8 and MMP-9 in both groups were increased,and the levels of study group were lower than those in the control group(P<0.05).After 3 months of repair,there were no statistical differences in PD,SBI,PLI,MMP-2,MMP-8 and MMP-9 between the two groups(P>0.05).The repair satisfaction of the study group was better than that of the control group(P<0.05).Conclusion CAD/CAM zirconia porcelain crown has good effect on dental crown,less stimulation to periodontal tissue,and high patient satisfaction.
