单孔腹腔镜阑尾切除术治疗急性阑尾炎的临床效果分析

摘　　要：目的:探究单孔腹腔镜阑尾切除术治疗急性阑尾炎的临床效果。方法:选取沂水县杨庄镇中心卫生院收治的100例急性阑尾炎患者(收治时间:2021年1月—2023年1月)作为研究对象,随机分为对照组(n=46)和观察组(n=54)。对照组实施传统多孔腹腔镜阑尾切除术,观察组实施单孔腹腔镜阑尾切除术。比较两组治疗效果。结果:观察组术中出血量少于对照组,排气时间、胃肠功能恢复时间早于对照组,手术时间、住院时间短于对照组,住院费用高于对照组(P<0.05)。术前,两组血清C反应蛋白(CRP)、白细胞介素-8(IL-8)、细胞间黏附分子1(ICAM-1)水平对比,无统计学差异(P>0.05);术后3 d,两组CRP、IL-8、ICAM-1水平均升高,但观察组低于对照组(P<0.05)。术后3个月,观察组切口美观总满意度高于对照组(P<0.05)。结论:单孔腹腔镜阑尾切除术治疗急性阑尾炎的临床效果较好,可减少术中出血量,减轻炎性反应,提高切口美观满意度,利于患者康复。Objective:To explore the clinical effect of single-port laparoscopic appendectomy in treatment of acute appendicitis.Methods:A total of 100 patients with acute appendicitis admitted to Yangzhuang Town Central Health Center in Yishui County(admission time:January 2021 to January 2023)were selected as the study subjects.They were randomly divided into control group(n=46)and observation group(n=54).The control group underwent traditional multi-port laparoscopic appendectomy,while the observation group underwent single-port laparoscopic appendectomy.The treatment effects of the two groups were compared.Results:The intraoperative blood loss in the observation group was less than that in the control group,the exhaust time and the gastrointestinal function recovery time in the observation group were earlier than those in the control group,the operation time and the length of hospital stay in the observation group were shorter than those in the control group,and the hospitalization expenses in the observation group was higher than that in the control group(P<0.05).Before operation,there was no significant difference in the levels of serum C-reactive protein(CRP),interleukin-8(IL-8)and intercellular adhesion molecule 1(ICAM-1)between the two groups(P>0.05).On 3 d after operation,the levels of CRP,IL-8 and ICAM-1 in the two groups increased,but the levels in the observation group were lower than those in the control group(P<0.05).At 3 months after operation,the total satisfaction with the incision aesthetics in the observation group was higher than that in the control group(P<0.05).Conclusion:Single-port laparoscopic appendectomy in treatment of acute appendicitis has good clinical effects,and can reduce intraoperative blood loss,relieve inflammatory reaction,improve the satisfaction with incision aesthetics,and facilitate patient's recovery.
