摘　　要：目的:分析飞秒激光小切口角膜基质透镜取出术(SMILE)和飞秒激光辅助准分子激光原位角膜磨镶术(FS-LASIK)矫治近视的临床效果。方法:选取2022年6月山东滨州沪滨爱尔眼科医院屈光手术科收治的60例(119眼)近视患者作为研究对象,随机分为研究组(30例,59眼)与对照组(30例,60眼)。研究组接受SMILE手术,对照组接受FS-LASIK手术。比较两组手术效果。结果:术后1、3个月,两组裸眼视力高于术前,球镜屈光度、柱镜屈光度、等效球镜度低于术前(P<0.05);两组间比较,无统计学差异(P>0.05)。术后3个月,研究组3 mm瞳孔直径下垂直彗差、球差大于对照组(P<0.05);两组水平彗差无明显变化(P>0.05)。术后3个月,两组总高阶像差无明显变化(P>0.05);两组斯特列尔比、调制传递函数值升高,且研究组大于对照组(P<0.05)。结论:SMILE和FS-LASIK矫治近视的临床效果均较好,能够改善患者视力与屈光度,其中SMILE可能会影响患者角膜高阶像差,但能改善患者全眼高阶像差,提高视觉质量。Objective:To analyze the clinical effect of femtosecond laser small incision lenticule extraction(SMILE)of corneal stroma and femtosecond laser-assisted excimer laser in-situ keratomileusis(FS-LASIK)in the correction of myopia.Methods:A total of 60 patients(119 eyes)with myopia who were admitted to Department of Refractive Surgery at Binzhou Hubin Aier Eye Hospital of Shandong Province in June 2022 were selected as the study subjects.They were randomly divided into study group(30 patients,59 eyes)and control group(30 patients,60 eyes).The study group received SMILE surgery,and the control group received FS-LASIK surgery.The surgical effect was compared between the two groups.Results:At 1 and 3 months after surgery,the naked vision in the two groups was higher than that before surgery,and the diopter of sphere,diopter of cylinder and spherical equivalent in the two groups were lower than those before surgery(P<0.05);There was no significant difference between the two groups(P>0.05).At 3 months after surgery,the vertical coma aberration and spherical aberration at 3 mm pupil diameter in the study group were greater than those in the control group(P<0.05).There was no significant change in the horizontal coma aberration between the two groups(P>0.05).At 3 months after surgery,there was no significant change in the total higher-order aberration between the two groups(P>0.05).The Strehl ratio and modulation transfer function value in the two groups increased,and these indicators in the study group were greater than those in the control group(P<0.05).Conclusion:SMILE and FS-LASIK have good clinical effects in the correction of myopia,and can improve patients'visual acuity and refraction.SMILE may affect the corneal higher-order aberration,but can improve the higher-order aberration of the whole eye and increase the visual quality.
