摘  要:目的:本研究旨在探讨^(68)Ga-成纤维细胞活化蛋白抑制剂(fibroblast activation protein inhibitor,FAPI)-04正电子发射体层成像(positron emission tomography,PET)/计算机体层成像(computed tomography,CT)显像的最佳扫描时间点,并评估肿瘤与阻塞性炎症中^(68)Ga-FAPI-04药代动力学的差异。方法:回顾并分析2021年1—11月因腹部肿瘤在海军军医大学第一附属医院接受^(68)Ga-FAPI-04 PET/CT显像的患者临床资料。利用Kruskal-Wallis H秩和检验比较各时间点肿瘤灶的最大标准摄取值(maximum standard uptake value,SUV_(max))、肿瘤背景比值(tumor-to-background ratio,TBR)和图像信噪比(signal to noise ratio,SNR);通过Mann-Whitney U检验对肿瘤及阻塞性炎症的滞留指数(retention index,RI)进行差异分析。结果:本研究共纳入31例患者,在15 min、1 h和2 h肿瘤灶的SUV_(max)分别为8.52(4.18~18.4)、8.59(4.69~13.8)和7.64(3.22~21.0),对应的TBR为4.22(1.87~17.76)、4.67(2.20~29.05)和4.30(2.48~35.20)。尽管1 h的SUV_(max)和TBR值高于其他时间点,但差异无统计学意义(P>0.05)。15 min、1 h和2 h的SNR分别为6.41(2.45~8.24)、5.0(1.68~5.57)和3.02(1.39~6.97),差异有统计学意义(H=51.691,P<0.001),其中15 min的SNR最高。此外,15 min与1 h间肿瘤灶的RI显著高于阻塞性炎症(-0.195~0.415 vs-0.422~0.115),差异亦有统计学意义(P=0.018)。结论:注射^(68)Ga-FAPI-04后15 min的图像SNR最高,可作为临床常规扫描时间点。此外,15 min与1 h的RI可有效地用于区分肿瘤与阻塞性炎症。Objective:To determine the optimal scanning time point for ^(68)Ga-fibroblast activation protein inhibitor(FAPI)-04 positron emission tomography(PET)/computed tomography(CT)imaging and to evaluate the differences in the pharmacokinetics of ^(68)Ga-FAPI-04 between tumors and obstructive inflammation.Methods:A retrospective analysis of clinical data from patients who underwent ^(68)Ga-FAPI-04 PET/CT imaging due to abdominal tumors in The First Affiliated Hospital of Naval Medical University between January and November 2021 was conducted.The maximum standard uptake value(SUV_(max)),tumor-to-background ratio(TBR),and signal-to-noise ratio(SNR)were compared across different time points using the Kruskal-Wallis H test,while the retention index(RI)for tumors and obstructive inflammation was analyzed using the Mann-Whitney U test.Results:A total of 31 patients were included in this study.The SUV_(max) values for tumor lesions at 15 min,1 h,and 2 h were 8.52(4.18–18.4),8.59(4.69–13.8),and 7.64(3.22–21.0),respectively,with corresponding TBR values of 4.22(1.87–17.76),4.67(2.20–29.05),and 4.30(2.48–35.20).Although the SUV_(max) and TBR at 1 h were higher than those at other time points,the differences were not statistically significant(P>0.05).The SNR values at 15 min,1 h,and 2 h were 6.41(2.45–8.24),5.0(1.68–5.57),and 3.02(1.39–6.97),showing significant differences(H value:51.691,P<0.001),with the highest SNR observed at 15 min.Furthermore,the RI for tumors at 15 min and 1 h was significantly higher than that for obstructive inflammation(-0.195–0.415 vs-0.422–0.115),with a statistically significant difference(P=0.018).Conclusion:The image SNR is highest at 15 min after the injection of ^(68)Ga-FAP1-04,making it a suitable time point for routine clinical scanning.Additionally,the RI at 15 min and 1 h is beneficial for distinguishing between tumors and obstructive inflammation.
