摘　　要：目的:研究小红细胞百分比(%Micro)与低色素性红细胞百分比(%Hypo)的比值对β-地中海贫血与缺铁性贫血(IDA)的鉴别诊断价值。方法:选取2021年6月—2023年5月贺州市人民医院明确诊断的β-地中海贫血患者100例(β-地中海贫血组)、IDA患者100例(IDA组)以及健康体检者50例(健康组)作为研究对象,均使用BC-6800 Plus全自动血细胞分析仪测定%Micro、%Hypo及%Micro/%Hypo比值(M/H)。比较三组%Micro、%Hypo值及M/H,通过受试者工作特征曲线分析不同Cut-off值下M/H的鉴别诊断效果。结果:β-地中海贫血组和IDA组%Micro均高于健康组(P<0.05);β-地中海贫血组和IDA组%Micro比较,差异无统计学意义(P>0.05)。β-地中海贫血组和IDA组%Hypo均高于健康组(P<0.05);β-地中海贫血组%Hypo低于IDA组(P<0.05)。β-地中海贫血组M/H高于IDA组(P<0.05)。当M/H≥5.915时,鉴别诊断效能最佳,灵敏度为79.0%,特异度为88.7%。结论:当M/H≥5.915时,对β-地中海贫血与IDA的鉴别诊断效能最佳,灵敏度为79.0%,特异度为88.7%,具有一定的鉴别诊断作用。Objective:To investigate the value of the ratio of microerythrocyte percentage(%Micro)to hypochromic erythrocyte percentage(%Hypo)in the differential diagnosis ofβ-thalassemia and iron deficiency anemia(IDA).Methods:A total of one hundred patients withβ-thalassemia(β-thalassemia group),one hundred patients with IDA(IDA group)and fifty healthy physical examination subjects(healthy group)were selected as the study objects from June 2021 to May 2023 in Hezhou People's Hospital.The BC-6800 Plus fully automatic blood cell analyzer were used to measure%Micro,%Hypo,and%Micro/%Hypo ratio(M/H).The values of%Micro,%Hypo and M/H of the three groups were compared,and the differential diagnosis effect of M/H under different Cut-off values was analyzed by receiver operating characteristic curve.Results:The%Micro ofβ-thalassemia group and IDA group was higher than that of healthy group(P<0.05);there was no significant difference in%Micro betweenβ-thalassemia group and IDA group(P>0.05).The%Hypo ofβ-thalassemia group and IDA group was higher than that of healthy group(P<0.05);the%Hypo ofβ-thalassemia group was lower than that of IDA group(P<0.05).The M/H inβ-thalassemia group was higher than that in IDA group(P<0.05).When M/H≥5.915,the differential diagnosis efficiency was the best,the sensitivity was 79.0%,and the specificity was 88.7%.Conclusion:When M/H≥5.915,the differential diagnosis ofβ-thalassemia and IDA is the best,the sensitivity is 79.0%,the specificity is 88.7%,and the differential diagnosis has a certain effect.
