摘 要:脉冲电场消融(PFA)技术作为一种非热物理消融手段,在肿瘤治疗中具有独特的优势。尽管PFA已被美国食品和药物管理局及我国药品监督管理局批准用于肿瘤治疗,并被中国临床肿瘤学会纳入胰腺癌的诊疗指南中,但其在临床应用中仍面临困境。本文系统性阐述国内外可用于肿瘤消融的脉冲源发生设备的研究进展、PFA治疗电极及消融术式的优化、PFA治疗胰腺癌的临床疗效及目前的研究现状,并对未来PFA技术治疗肿瘤的攻克方向提出了建议,以期促进PFA在胰腺癌及其他难治性肿瘤的临床应用。Pulsed field ablation(PFA)is a non-thermal physical ablation technique that offers unique advantages in tumor treatment.Although PFA has been approved by the Food and Drug Administration and the National Medical Products Administration of the USA and China for tumor treatment,and included in the clinical guidelines for pancreatic cancer by the Chinese Society of Clinical Oncology,it still faces challenges in clinical oncology practice.This article systematically reviews the research progress on pulse generation devices applicable to tumor ablation,optimization of PFA electrodes and ablation operation,clinical efficacy and current research state of PFA in treating pancreatic cancer.Additionally,it proposes directions for future studies on PFA technology in tumor treatment,aiming to promote its clinical application in pancreatic cancer and other refractory tumors.
