摘　　要：目的探讨Solitaire AB支架机械介入取栓术联合阿替普酶溶栓治疗急性缺血性脑卒中的效果及对脑部血流速度、神经因子水平的影响。方法选取2020年1月至2022年12月收治的100例急性缺血性脑卒中患者为研究对象,根据入院时间将其分为对照组和观察组,各50例。对照组接受阿替普酶溶栓治疗,观察组在对照组基础上加Solitaire AB支架机械介入取栓术。比较两组的治疗效果。结果观察组的治疗总有效率为96.00%,明显高于对照组的82.00%(P<0.05)。治疗后,观察组的大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)血流速度均大于对照组(P<0.05)。治疗后,观察组的神经元特异性烯醇化酶(NSE)、髓鞘碱性蛋白(MBP)水平低于对照组,神经生长因子(NGF)、脑源性神经营养因子(BDNF)水平高于对照组(P<0.05)。结论Solitaire AB支架机械介入取栓术联合阿替普酶溶栓治疗急性缺血性脑卒中的效果显著,可改善脑部血流速度,调节神经因子水平,值得推广。Objective To investigate the effect of Solitaire AB stent mechanical interventional thrombectomy combined with alteplase thrombolysis on acute ischemic stroke and its influences on cerebral blood flow velocity and nerve factors levels.Methods A total of 100 patients with acute ischemic stroke admitted from January 2020 to December 2022 were selected as the research objects.According to the time of admission,the patients were divided into control group and observation group,with 50 cases in each group.The control group was treated with alteplase thrombolysis,and the observation group was treated with Solitaire AB stent mechanical interventional thrombectomy on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 96.00%,which was significantly higher than 82.00%in the control group(P<0.05).After treatment,the blood flow velocities of middle cerebral artery(MCA),anterior cerebral artery(ACA)and posterior cerebral artery(PCA)in the observation group were larger than those in the control group(P<0.05).After treatment,the levels of neuron specific enolase(NSE)and myelin basic protein(MBP)in the observation group were lower than those in the control group,and the levels of nerve growth factor(NGF)and brain-derived neurotrophic factor(BDNF)were higher than those in the control group(P<0.05).Conclusion Solitaire AB stent mechanical interventional thrombectomy combined with alteplase thrombolysis in the treatment of acute ischemic stroke has a significant effect,it can improve the blood flow velocity of the brain and regulate the levels of nerve factors,which is worthy of promotion.
