摘 要:目的采用气相质谱联用(GC-MS)法探讨慢传输型便秘(STC)患者粪便短/中链脂肪酸(SCFAs/MCFAs)的变化及其与便秘严重程度的关系。方法收集福建中医药大学附属第二人民医院30例STC组和30例正常组研究对象的新鲜粪便,采用GC-MS法分析粪便SCFAs/MCFAs情况,并探讨其与便秘严重度评分(CSS)的关系。结果共检测到9种SCFAs,4种MCFAs,STC组总SCFAs水平显著低于正常组(Z=4.355,P<0.001),2组总MCFAs水平差异无统计学意义(Z=-1.028,P=0.304)。正交偏最小二乘法判别分析(OPLS-DA)显示2组SCFAs/MCFAs存在差异,且该评估模型稳定(Q^(2)=0.446,P<0.005),单变量分析结果显示STC患者与正常组相比,粪便中乙酸、丙酸、丁酸水平下调,己酸、庚酸水平上调。KEGG分析发现,2组差异性代谢物共涉及16条通路,其中STC组碳水化合物消化与吸收通路显著低于正常组(P=0.018)。相关性分析结果显示乙酸、丙酸、丁酸水平与CSS呈负相关,己酸、庚酸水平与CSS呈正相关。结论STC患者粪便SCFAs/MCFAs水平与健康人存在差异,表现为总SCFAs、乙酸、丙酸、丁酸水平降低,己酸、庚酸水平升高。Objective To observe the changes of fecal short/medium chain fatty acids(SCFAs/MCFAs)and their relationship with the severity of constipation in patients with slow-transmission constipation(STC)using gas chromatographymass spectrometry(GC-MS).Methods Fresh feces from 30 cases in STC group and 30 cases in normal control group in the hospital were collected,the fecal SCFAs/MCFAs were analyzed by using GC-MS,and their relationship with constipation severity score(CSS)was observed.Results A total of 9 SCFAs and 4 MCFAs were detected.The total SCFAs level in the STC group was significantly lower than that in the normal control group(Z=4.355,P<0.001),while the difference in the total MCFAs levels between the two groups was not statistically significant(Z=−1.028,P=0.304).Orthogonal partial least squares discriminant analysis(OPLS-DA)showed that there were differences in s SCFAs/MCFAs between the two groups,and this assessment model was stable(Q^(2)=0.446,P<0.005).The results of univariate analysis showed that the levels of fecal acetic,propionic and butyric acids were down-regulated,while those of hexanoic acid and heptanoic acid were up-regulated in the STC patients compared with those in the normal control group.KEGG analysis revealed that the differential metabolites in the two groups involved 16 pathways,in which the carbohydrate digestion and absorption pathways were significantly lower in the STC group than in the normal control group(P=0.018).Correlation analysis showed that the levels of acetic acid,propionic acid and butyric acid were negatively correlated with CSS,while heptanoic acid and hexanoic acid were positively correlated with CSS.Conclusion Fecal SCFAs/MCFAs levels in STC patients differed from those of healthy individuals,as shown by a decrease in the levels of total SCFAs,acetic acid,propionic acid and butyric acid,and an increase in the levels of hexanoic acid and heptanoic acid.
