摘  要:目的探讨二氧化碳(carbon dioxide,CO_(2))冷冻疗法对不同分期肺鳞状细胞癌(pulmonary squamous cell carcinoma,SqCLC)患者肿瘤微环境免疫细胞浸润的影响。方法选取2020年10月至2023年10月唐山市人民医院收治的106例SqCLC患者作为研究对象,按照治疗方法分为研究组(n=53)和对照组(n=53)。对照组采取全身化疗治疗,研究组在化疗的基础上联合支气管镜CO_(2)冷冻疗法进行治疗。比较不同治疗方法对不同分期SqCLC患者免疫细胞浸润的影响,并观察近期治疗效果。结果治疗前,≥65岁患者CD8^(+)T淋巴细胞水平显著高于<65岁患者(P<0.05);治疗后,研究组患者CD3^(+)T淋巴细胞、CD4^(+)T淋巴细胞水平均显著高于对照组(均P<0.05),且较治疗前均显著升高(均P<0.05);研究组患者CD8^(+)T淋巴细胞、Treg细胞水平均显著低于对照组(均P<0.05),且较治疗前均显著降低(均P<0.05);治疗后,Ⅲ期组和Ⅳ期组患者CD3^(+)T淋巴细胞、CD4^(+)T淋巴细胞水平均显著高于治疗前(均P<0.05),CD8^(+)T淋巴细胞、Treg细胞水平均显著低于治疗前(均P<0.05);研究组患者总有效率显著高于对照组(P<0.05)。广义估计方程(generalized estimating equation,GEE)模型分析可知,研究组患者CD3^(+)T淋巴细胞、CD4^(+)T淋巴细胞、CD8^(+)T淋巴细胞、Treg细胞改善情况均显著优于对照组(均P<0.05)。结论支气管镜下CO_(2)冷冻疗法可破坏肿瘤细胞,影响肿瘤微环境免疫细胞浸润,调节免疫功能,提升治疗效果。Objective To investigate the effect of carbon dioxide(CO_(2))cryotherapy on immune cell infiltration in the tumor microenvironment of patients with different stages of pulmonary squamous cell carcinoma(SqCLC).Method 106 patients with SqCLC patients admitted to Tangshan People's Hospital from October 2020 to October 2023 were selected as the study Subjects and were divided into the study group(n=53)and control group(n=53)according to the treatment method.The control group received systemic chemotherapy,while the study group received combined bronchoscopic CO_(2)cryotherapy on the basis of chemotherapy;the effects of different treatment methods on immune cell infiltration in patients with SqCLC at different stages were compared,and the short-term treatment effects were observed.Result Before treatment,the level of CD8^(+)T lymphocyte in patients aged≥65 was significantly higher than that in patients aged<65(P<0.05).After treatment,the levels of CD3^(+)T lymphocyte and CD4^(+)T lymphocyte in patients of the study group were significantly higher than those in the control group(all P<0.05),and were significantly higher than before treatment(all P<0.05).The levels of CD8^(+)T lymphocyte and Treg cells in the study group were significantly lower than those in the control group(all P<0.05),and were significantly lower than before treatment(all P<0.05).After treatment,the levels of CD3^(+)T lymphocyte and CD4^(+)T lymphocyte in patients in stageⅢgroup andⅣgroup were significantly higher than those before treatment(all P<0.05),and the levels of CD8^(+)T lymphocyte and Treg cells were significantly lower than those before treatment(all P<0.05).The total effective rate in the study group was significantly higher than that in the control group(P<0.05).Generalized estimating equation(GEE)analysis showed that the improvement of CD3^(+)T lymphocyte,CD4^(+)T lymphocyte,CD8^(+)T lymphocyte,and Treg cells in the study group was significantly better than that in the control group(all P<0.05).Conclusion Bronchoscopic CO_(2)cryo
