基于数据挖掘探析针刺治疗老年高血压的选穴配伍规律

摘　　要：目的探讨针刺治疗老年高血压的临床选穴规律。方法检索中国期刊全文数据库(CNKI)、万方数据知识服务平台(Wangfang)、维普资讯中文期刊服务平台(VIP)、中国生物医学文献数据库(Sino Med)及Pub Med数据库从建库至2023年1月收录的针刺治疗老年高血压的临床研究文献,运用数据挖掘技术对选取的腧穴、经络进行频次统计、关联规则分析及聚类分析。结果共检索到相关文献711篇,筛选后最终得到65篇,涉及腧穴70个,涉及经络15条。使用频次最高的腧穴依次是三阴交、曲池、百会、足三里、太冲、合谷、风池、内关、神门、太溪,常用经络是大肠经、督脉和胃经。关联规则分析显示关联度最大的是“三阴交-足三里-曲池”;聚类分析得到3个有效聚类群:神庭-印堂-内关,三阴交-百会-风池-曲池-合谷-足三里-神门,太冲-丰隆-太溪-肾俞。结论老年高血压多为本虚标实,与肝脾肾三脏功能失调有关,针刺治疗以补益脾胃,滋肾平肝,调畅气机为主,并采取远近配穴、表里配穴等多种配穴方法,以“太冲、合谷、曲池、三阴交、足三里、百会”为核心处方。Objective To explore the clinical acupoint selection rules for acupuncture treatment of elderly hypertension.Methods Retrieve clinical research literature on acupuncture treatment for elderly hypertension from the CNKI,Wanfang Data Knowledge Service Platform(Wangfang),VIP Chinese Journal Service Platform,Sino Med,and Pub Med databases from their establishment to January 2023.Use data mining techniques to perform frequency statistics,association rule analysis,and cluster analysis on the selected acupoints and meridians.Results A total of 711 relevant literature were retrieved,and after screening,65 articles were finally obtained,involving 70 acupoints and 15 meridians.The most frequently used acupoints are Sanyinjiao,Quchi,Baihui,Zusanli,Taichong,Hegu,Fengchi,Neiguan,Shenmen,and Taixi.The commonly used meridians are the large intestine meridian,Du meridian,and stomach meridian.The analysis of association rules shows that the highest degree of association is“Sanyinjiao-Zusanli-Quchi”;Cluster analysis identified three effective clusters:Shenting-Yintang-Neiguan,Sanyinjiao-Baihui-Fengchi-Quchi-Hegu-Zusanli-Shenmen,and Taichong-Fenglong-Taixi-Shenshu.Conclusion Hypertension in the elderly is mostly caused by deficiency in the root and excess in the target,which is related to the dysfunction of the liver,spleen,and kidney.Acupuncture treatment mainly aims to nourish the spleen and stomach,nourish the kidneys and calm the liver,and regulate Qi flow.Multiple acupoint matching methods such as near and far matching,exterior and interior matching,etc.are adopted,with“Taichong,Hegu,Quchi,Sanyinjiao,Zusanli,Baihui”as the core prescription.
