摘　　要：基于关系空间理论对家庭病床服务中医患沟通特点及其关系进行探讨。家庭病床服务中的医患互动在患者熟悉且充满地方感的家庭中展开,适应老年患者传统思维和家庭照护习惯的诊疗行为更容易为患者所接受。“在场”的情境化沟通促使患者的认知与现代医学知识相融,减少患者及其照护者的认知偏差,实现居家情境与康复行为的互动。在医患长期互动与互惠中,工具性的医患关系发展为情感性的熟人关系。这种良性关系构建不仅有助于医患健康共识的形成、疾病的缓解甚至治愈,也是社会认同的重要组成部分。Based on the theory of relational space,this paper discusses the communication characteristics and relationship of doctor-patient in home hospitalization services.The doctor-patient interaction in home hospitalization services is carried out in the family that the patients are familiar with and full of sense of place,and the diagnosis and treatment behavior that adapts to the traditional thinking of the elderly patients and family care habits is more easily accepted by the patients.The situational communication of"presence"promotes the integration of patients'cognition and modern medical knowledge,reduces cognitive bias of patients and their caregivers,and realizes the interaction between home situation and rehabilitation behavior.In the longterm interaction and reciprocity between doctors and patients,the instrumental doctor-patient relationship has developed into an emotional acquaintance relationship.This positive relationship construction not only aids in forming a mutual health consensus and in alleviating or curing illnesses but also plays an important role in social identification.
