摘　　要：目的了解不可复性关节盘前移位(ADDWOR)患者恐惧疾病现状以及生活质量的情况,深入分析这两者之间的相关性。方法采用口腔健康影响程度量表-14中文版(OHIP-14)及恐惧疾病进展简化量表(FoP-Q-SF)对2021年4月—2022年4月在天津市口腔医院颞颌病门诊就诊的200例ADDWOR患者进行问卷调查并分析患者口腔健康影响程度总分与恐惧疾病进展各层面相关性。结果200例患者口腔健康影响程度总分为(23.57±3.48)分,生理疼痛分数最高,其次是生理障碍得分。恐惧疾病进展的总分为(28.95±4.82)分,生理健康层面得分为(17.53±3.63)分,社会家庭层面得分为(11.43±2.82)分。ADDWOR患者恐惧疾病进展其生理健康层面和社会家庭层面均与口腔健康影响程度总分呈正相关,且恐惧疾病进展总分与口腔健康影响程度总分相关系数为0.625。结论ADDWOR患者大多有恐慌、焦虑等方面的负性情绪,病情发展的恐慌状况直接关系到患者的生活质量水平,因此应给予不可复性关节盘前移位患者有效的干预措施,以降低恐惧疾病进展程度,提高生活质量。Objective To understand the status of fear disease and quality of life in patients with anterior disc displacement without reduction(ADDWOR),and deeply analyze the correlation between the two.Methods A total of 200 patients with ADDWOR treated in the temporomandibular disease clinic of Tianjin Stomatology Hospital from April 2021 to April 2022 were surveyed by using the Oral Health Impact Profile-14(OHIP-14)and the Fear of Progression Questionnaire-Short Form(FoP-Q-SF),and the correlation between the total score of oral health impact degree and various levels of fear of disease progression were analyzed.Results Total score of oral health impact degree of 200 patients was(23.57±3.48)points,physiological pain score was highest;followed by the score of physiological disorder.The total score of fear of disease progression was(28.95±4.82)points,physiological health aspect score was(17.53±3.63)points,the score of social family aspect was(11.43±2.82)points.The physiological health aspect and social family aspect of fear disease progression of patients with ADDWOR were positively correlated with the total score of oral health impact degree,the correlation coefficient between the total score of fear disease progression and the total score of oral health impact degree was 0.625.Conclusion Most patients with ADDWOR had negative emotions such as panic and anxiety.The panic of disease development was directly related to quality of life of patients.Therefore,effective intervention measures should be provided to patients with irretrievable anterior articular disc displacement to reduce fear degree of disease progression and imporve quality of life.
