摘 要:目的:比较1%聚维酮碘(PVP-I)、2.5%次氯酸钠(NaClO)和生理盐水对感染根管内粪肠球菌(E.f)的抗菌作用,探究PVP-I作为难治性根尖周炎根管冲洗液的可行性。方法:体外培养浮游态及生物膜的E.f,根据冲洗液不同分为3组:1%PVP-I组、2.5%NaClO组和生理盐水组(n=6)。分别使用上述3种冲洗液处理后,通过测定生长曲线和菌落计数,比较各组冲洗液对浮游态及生物膜态E.f生长的影响作用。收集18个因正畸治疗需要拔除的牙齿,经鉴定均为单根下颌前磨牙,制备牙根模型并通过接种E.f制备感染根管模型。分别使用上述3种冲洗液处理后,通过扫描电镜(SEM)及激光扫描共聚焦显微镜(CLSM)定性评价冲洗液对根管内E.f生物膜的抗菌效果。结果:1%PVP-I和2.5%NaClO对两种状态E.f的抑制效果明显强于生理盐水冲洗组(P<0.05),但1%PVP-I组和2.5%NaClO组间无统计学差异(P>0.05)。SEM观察结果显示,1%PVP-I和2.5%NaClO能有效抑制根管表面E.f的定植。CLSM观察结果显示,2.5%NaClO和1%PVP-I均能够有效抑制牙本质小管一定深度内E.f的生长活性。结论:1%PVP-I可以有效抑制根管内E.f的定植,为临床中难治性根尖周炎的根管冲洗提供新的思路和方法。AIM:Compare the antibacterial effects of 1%PVP-I,2.5%NaClO,and physiological saline on the Enterococcus faecalis in infected root canals,and explore the feasibility of using povidone iodine as root canal irrigation solution in the refractory apical periodontitis.METHODS:The Enterococcus faecalis cultured in vitro in both planktonic and biofilm states.The study was divided into three groups:1%PVP-I group,2.5%NaClO group,and physiological saline group(n=6).After treating with the three types of irrigation solutions mentioned above,the effects of each group on the growth of planktonic Enterococcus faecalis or biofilm were compared by measuring growth curves and colony forming unit counts.18 single mandibular premolars extracted during orthodontic treatment were collected,prepared as root model,and infected by inoculating with Enterococcus faecalis.After treating with the three types of irrigation solutions mentioned above,the antibacterial effects of each group of irrigation solutions on the biofilm of Enterococcus faecalis in the root canal were observed by SEM and CLSM.RESULTS:The antibacterial effect of 1%PVP-I and 2.5%NaClO on the growth of Enterococcus faecalis in planktonic and biofilm states was significantly stronger than that of the physiological saline group(P<0.05),but there was no statistical difference between the 1%PVP-I group and the 2.5%NaClO group(P>0.05).SEM observation results show that 1%PVP-I and 2.5%NaClO can effectively inhibit the colonization of Enterococcus faecalis on the surface of root canals.CLSM observation results show that 2.5%NaClO and 1%PVP-I are available to inhibit the growth activity of Enterococcus faecalis colonizing in the depth of dentinal tubules.CONCLUSION:1%PVP-I successfully inhibit the colonization of Enterococcus faecalis resident in the root canal,and can be used as irrigation solutions for infected root canals in the refractory apical periodontitis in clinical practice.
