摘  要:目的探究儿童抽动障碍(TD)的临床特征、抽动严重程度与脑电图(EEG)的关系。方法选取符合TD诊断患儿92例进行分型,用耶鲁综合抽动严重程度量表(YGTSS)评分、30 min视频脑电图(VEEG)检查,分析3种分型的TD患儿的临床症状、抽动的严重程度,以及EEG的特征。结果92例TD患儿短暂性抽动(TTD)55例,慢性运动性或发声性抽动(CTD)16例,多发性抽动(TS)21例,EEG正常77例,异常12例,界限3例,3种类型TD患儿年龄、性别、有无合并症比较差异均无统计学意义(P>0.05),其中TS型YGTSS评分、EEG阳性率、抽动严重程度要高于TTD和CTD,有合并症的TD患儿抽动程度重于无合并症患儿。结论TD患儿有较高的EEG阳性率,EEG异常与临床分型、抽动严重程度密切相关,多集中在双侧颞区,以棘慢波、尖波、慢波发放为主,可为临床TD诊断分型、病情分析及评估预后提供客观依据。Objective The purpose of this study is to investigate the clinical characteristics and severity of TD in children,as well as their relationship with electroencephalography(EEG).Methods 92 pediatric patients who met the diagnosis of TD were selected for classification.The YGTSS score and a 30 min video electroencephalogram(VEEG)examination were utilized to analyze the clinical symptoms,tic severity,and EEG characteristics of the three types of TD patients.Results There were 55 cases of TTD,16 cases of CTD,21 cases of TS,77 cases with normal EEG results,12 cases with abnormal EEG results,and a maximum of 3 cases in 92children with TD,there were no statistically significant differences in age,gender,and the presence or absence of comorbidities among the three types of TD patients(P>0.05).Among them,the TS type YGTSS score,EEG positivity rate,and tic severity were higher than those of TTD and CTD.Children with TD and comorbidities exhibited more severe tic symptoms compared to those without comorbidities.Conclusions Children with TD exhibit a higher incidence of positive EEG results.EEG abnormalities are strongly associated with the clinical classification and severity of tics.These abnormalities are predominantly found in the bilateral temporal region,characterized by spike slow waves,sharp waves,and slow wave discharges.This can provide an objective basis for clinical diagnosis and classification of TD,disease analysis,and evaluation of prognosis.
