肿瘤异常糖链糖蛋白联合中性粒细胞淋巴细胞比率对结直肠癌的诊断价值

摘　　要：目的探讨肿瘤异常糖链糖蛋白(TAP)联合中性粒细胞淋巴细胞比率(NLR)对结直肠癌的诊断价值。方法选取2023年1月至2024年1月在九江市第一人民医院病理确诊为结直肠癌并接受手术的80例患者为研究组,同时收集同期80名健康体检人群为对照组。分析研究组与对照组TAP、NLR的检测值,评估不同病理特征及直肠癌患者TAP、NLR表达水平,绘制受试者工作特征(ROC)曲线分析TAP、NLR对结直肠癌的诊断价值。结果研究组TAP、NLR水平高于对照组,差异有统计学意义(P<0.05)。肿瘤分化结直肠癌患者TAP、NLR水平高于未分化患者,低分化患者TAP、NLR水平高于中、高分化患者,中分化患者TAP、NLR水平高于高分化患者;有淋巴结转移结直肠癌患者TAP、NLR水平高于无淋巴结转移患者;Ⅲ期结直肠癌患者TAP、NLR水平高于Ⅰ期、Ⅱ期患者,Ⅱ期患者TAP、NLR水平高于Ⅰ期患者,差异均有统计学意义(P<0.05)。外周血TAP、NLR水平诊断结直肠癌的曲线下面积分别为0.883、0.875,联合诊断的曲线下面积为0.939,且联合诊断的曲线下面积高于各指标单独诊断。结论TAP联合NLR检测可有效提高结直肠癌的诊断价值,且TAP、NLR水平与肿瘤分化程度、淋巴结转移、TNM分期有关。Objective To explore the diagnostic value of tumor abnormal protein(TAP)combined with neutrophil lymphocyte ratio(NLR)in colorectal cancer.Methods A total of 80 patients pathologically confirmed with colorectal cancer and 80 healthy controls in Jiujiang First People's Hospital were enrolled as study group and control group between January 2023 and January 2024.TAP and NLR in both groups were analyzed.The expression levels of TAP and NLR in patients with different pathological characteristics and colorectal cancer were evaluated.The diagnostic value of TAP and NLR for colorectal cancer were analyzed by receiver operating characteristic(ROC)curves.Results TAP and NLR were higher in study group than control group,the differences were statistically significant(P<0.05).The levels of TAP and NLR in patients with differentiated colorectal cancer were higher than those in patients with undifferentiated tumors,the levels of TAP and NLR in poorly differentiated colorectal cancer were higher than those in moderately and well differentiated colorectal cancer,and the levels of TAP and NLR in moderately differentiated colorectal cancer were higher than those in well differentiated colorectal cancer,the levels of TAP and NLR in colorectal cancer patients with lymph node metastasis were higher than those in patients without lymph node metastasis,the levels of TAP and NLR in patients with stageⅢcolorectal cancer were higher than those in stageⅠand stageⅡpatients,and the levels of TAP and NLR in stageⅡpatients were higher than those in stageⅠpatients,the differences were statistically significant(P<0.05).The area under the curve(AUC)values of TAP,NLR and combined detection in the diagnosis of colorectal cancer were 0.883,0.875 and 0.939,respectively.AUC of combined detection was higher than that of single index.Conclusion TAP combined with NLR can effectively improve diagnostic value in colorectal cancer.TAP and NLR are correlated with tumor differentiation degree,lymph node metastasis and TNM staging.
