摘　　要：目的探讨治疗性沟通系统干预对消化道出血患者疾病不确定感、情绪及希望水平等的影响。方法将96例消化道出血患者随机分为A组和B组,每组48例。A组患者接受常规护理干预,B组患者在A组基础上接受治疗性沟通系统干预,两组患者均干预住院全程。比较干预前后两组患者Mishel疾病不确定感量表(MUIS-A)、焦虑自评量表(SAS)、抑郁自评量表(SDS)、Herth希望量表(HHI)、自我护理能力量表(ESCA)评分。结果干预后两组患者MUIS-A各维度、SAS、SDS评分较干预前均降低,且B组低于A组(P<0.05或0.01)。干预后两组患者HHI、ESCA各维度评分较干预前均升高,且B组高于A组(P<0.05或0.01)。结论治疗性沟通系统干预能够减轻消化道出血患者疾病不确定感,使患者保持良好的心理状态,提升内心希望水平,并提高患者自我护理能力。Objective To explore the effect of therapeutic communication system(TCS)intervention on illness uncertainty,emotions and hope level in patients with gastrointestinal bleeding(GIB).Methods Ninety-six GIB patients were randomly divided into group A and B,with 48 ones in each.Group A received routine nursing,B did the TCS intervention on the basis of group A,and both groups were intervened in the whole course of hospitalization.Pre-and post-intervention scores on the Mishel Uncertainty in Illness Scale(MUIS-A),Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),Her th Hope Index(HHI),and Exercise of Self-Care Agency Scale(ESCA)were compared between the two groups.Results After intervention,each dimension scores of the MUIS-A as well as SAS and SDS scores in both groups lowered compared with pre-intervention and those were lower in group B than A(P<0.05 or 0.01).After intervention each dimension scores of both the HHI and ESCA in both groups elevated compared with pre-intervention and those were higher in group B than A(P<0.05 or 0.01).Conclusion The TCS intervention can alleviate the illness uncertainty of GIB patients,helps them maintain good psychological status,promote their inner level of hope,and improve their self-care ability.
