靳三针及择时穴位按摩联合身痛逐瘀汤治疗中风后痉挛性偏瘫临床研究

摘　　要：目的:观察靳三针及择时穴位按摩结合身痛逐瘀汤治疗中风后痉挛性偏瘫的临床疗效。方法:选取2020年3月—2023年3月漯河市第二人民医院收治的中风后痉挛性偏瘫患者,共90例,采用简单随机分组法分为观察组和对照组各45例。对照组给予身痛逐瘀汤治疗;观察组给予靳三针及择时穴位按摩联合身痛逐瘀汤治疗。评价2组临床疗效,比较2组治疗前后中医证候积分、痉挛指数(CSI)、改良Ashworth量表(MAS)评分、功能综合评定量表(FCA)评分、简式Fugl-Meyer(FMA)量表评分、日常生活活动能力(Barthel)评分。结果:治疗3周后,2组中医证候各项症状包括半身不遂、感觉减退、口舌歪斜、气短乏力、面色㿠白、自汗出积分均低于治疗前,差异均有统计学意义(P<0.05)。观察组中医证候各项症状积分均低于对照组,差异均有统计学意义(P<0.05)。治疗1周后、治疗3周后,2组CSI、MAS评分均低于治疗前,FCA、FMA、Barthel评分均高于治疗前,差异均有统计学意义(P<0.05);观察组治疗1周后、治疗3周后CSI、MAS评分均低于对照组,FCA、FMA、Barthel评分均高于对照组,差异均有统计学意义(P<0.05)。治疗3周后,临床疗效总有效率观察组93.33%,对照组80.00%,差异有统计学意义(P<0.05)。结论:靳三针及择时穴位按摩联合身痛逐瘀汤治疗中风后痉挛性偏瘫能改善半身不遂、感觉减退,缓解痉挛,降低肌张力,促进肢体活动能力及自主生活能力的恢复。Objective:To observe the clinical effect of the combination use of Jin's Three-Needle and timing acupoint massage and Shentong Zhuyu Decoction on post-stroke spastic hemiplegia.Methods:A total of 90 patients with post-stroke spastic hemiplegia admitted to Luohe Second People's Hospital from March 2020 to March 2023 were selected and divided into the observation group and the control group by the simple random grouping method,with 45 cases in each group.The control group was treated with Shentong Zhuyu Decoction;the observation group was treated with Jin's three-needle and timing acupoint massage combined with Shentong Zhuyu Decoction.The clinical effects in the two groups were evaluated.The traditional Chinese medicine syndrome scores,Clinical Spasticity Index(CSI),and scores of Modified Ashworth Scale(MAS),Functional Comprehensive Assessment(FCA)scale,Fugl-Meyer Assessment(FMA)scale and Barthel Index(BI)were compared before and after treatment between the two groups.Results:After three weeks of treatment,the traditional Chinese medicine syndrome scores of hemiplegia,decreased sensation,mouth and tongue deviation,shortness of breath and fatigue,pale complexion and spontaneous sweating in the two groups were lower than those before treatment,the difference being significant(P<0.05).The above traditional Chinese medicine syndrome scores in the observation group were lower than those in the control group,differences being significant(P<0.05).After one week and three weeks of treatment,the scores of CSI and MAS in the two groups were lower than those before treatment,and the scores of FCA,FMA and BI were higher than those before treatment,differences being significant(P<0.05).After one week and three weeks of treatment,the CSI and MAS scores in the observation group were lower than those in the control group,and the scores of FCA,FMA and BI were higher than those in the control group,differences being significant(P<0.05).After three weeks of treatment,the total clinical effective rate was93.33%in the observation grou
