摘 要:目的:探讨三维CT支气管血管成像(3D-CTBA)联合3D打印技术在早期非小细胞肺癌(NSCLC)患者胸腔镜肺段切除术中的应用效果。方法:回顾分析2022年1月至2023年9月于本院行胸腔镜肺段切除术的87例早期NSCLC患者临床和影像学资料,其中术前采取常规CT检查患者44例(常规组)、3D-CTBA联合3D打印技术检查患者43例(联合组),以手术结果为标准,比较2组各指标的差异。结果:常规组和联合组的病变定位准确率分别为95.45%(42/44)、100%(43/43),差异无统计学意义(χ^(2)值=2.001,P值=0.157)。与常规组相比,联合组的术中出血量、胸管引流量降低(P<0.05),切缘宽度增加(P<0.05),手术时间、胸管留置时间、住院时间缩短(P<0.05)。联合组术后并发症发生率为13.95%,低于常规组的31.82%(P<0.05)。结论:使用3D-CTBA联合3D打印技术有助于提高早期NSCLC患者胸腔镜肺段切除术效果,包括准确定位病灶、缩短手术和住院时间、降低术中出血量和术后并发症发生率等。Purpose:To explore the application value of three-dimensional CT bronchial angiography(3D-CTBA)combined with 3D printing technology in thoracoscopic segmentectomy for patients with early non-small cell lung cancer(NSCLC).Methods:A retrospective analysis was performed on clinical and imaging data of 87 patients with early NSCLC who underwent thoracoscopic segmentectomy in the hospital between January 2022 and September 2023.Among the 87 patients,there were 44 cases in routine group(routine CT examination before surgery)and 43 cases in combination group(3D-CTBA combined 3D printing technology).Taking surgical results as the golden standard,differences in different indexes between the two groups were compared.Results:The accuracy rates of lesions localization in routine group and combination group were 95.45%(42/44)and 100%(43/43),respectively,with no statistically significant difference(χ^(2)=2.001,P=0.157).Compared with routine group,the intraoperative blood loss and drainage volume of thoracic tube were reduced(P<0.05),the width of resection margin was increased(P<0.05),and the operation time,indwelling time of thoracic tube and length of hospital stay were shortened(P<0.05)in combination group.The incidence of postoperative complications in combination group was lower than that in routine group(13.95%vs 31.82%,P<0.05).Conclusion:3D-CTBA combined with 3D printing technology is beneficial to improve thoracoscopic segmentectomy effect in early NSCLC,including accurately locating lesions,shortening operation time and length of hospital stay,reducing intraoperative blood loss and incidence of postoperative complications.
