复合颏下皮瓣修复面部软组织缺损的临床与美学效果分析

摘　　要：目的:探讨复合颏下皮瓣修复面部软组织缺损的临床与美学效果。方法:选取2020年3月-2022年2月于笔者医院收治面部缺损的患者118例为研究对象,根据手术方法(皮瓣移植部位)分为两组。对照组59例接受前臂游离皮瓣修复,观察组59例接受复合颏下皮瓣修复。对比两组手术相关指标、术后功能恢复情况、皮瓣修复美观性及美观满意度。结果:观察组总手术时间、皮瓣制作+吻合时间均较对照组短,术中出血量较对照组少,皮瓣Ⅰ期存活率均高于对照组(P<0.05);两组组织愈合、五官功能恢复时间对比差异无统计学意义(P>0.05);观察组受区色泽、质地及形态轮廓美学评分均较对照组高(P<0.05);观察组术后美观满意度(94.92%)较对照组(83.05%)高(P<0.05)。结论:复合颏下皮瓣修复用于面部软组织缺损患者中能缩短手术用时,提高皮瓣I期存活率,改善皮瓣修复功能性和美观性,提高美观满意度。Objective To investigate the functional and aesthetic properties of composite submental flap for facial defect.Methods A total of 118 patients with facial defects admitted to the author's hospital from March 2020 to February 2022 were selected as the study objects and divided into 2 groups according to the surgical method(flap transplantation site).The control group(59 cases)received forearm free flap repair,and the observation group(59 cases)received composite submental flap repair.The postoperative functional recovery,aesthetics and satisfaction of skin flap repair were compared between the two groups.Results The total operation time,flap fabrication and anastomosis time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the survival rate of the flap in stageⅠwas higher than that of the control group(P<0.05).There was no significant difference in the time of tissue healing and facial function recovery between the two groups(P>0.05).The aesthetic scores of color,texture and contour of the receiving area in the observation group were higher than those in the control group(P<0.05).The aesthetic satisfaction of observation group(94.92%)was higher than that of control group(83.05%)(P<0.05).Conclusion The composite submental flap can shorten the operation time,increase the survival rate of the flap in stage I,improve the functional and aesthetic features of the flap,and improve aesthetic satisfaction in the patients with facial defects.
