摘　　要：目的明确痤疮患者继发敏感性皮肤的发生现状和影响因素,为防治痤疮患者继发敏感性皮肤提供指导。方法采用分层随机抽样,对全国76家皮肤科参与“痤疮周”义诊的痤疮患者(n=5787)继发敏感性皮肤的情况进行横断面调查,通过总结人口学信息、痤疮严重程度、皮肤类型、家族史、日常行为、医疗行为、环境因素、地区等资料,明确痤疮患者继发敏感性皮肤的影响因素。结果5787例中,女3979例(68.76%),男1808例(31.24%),平均年龄(25.63±6.66)岁。痤疮继发敏感性皮肤共1840例,检出率为31.80%。Logistic回归分析显示:女性,痤疮严重程度,皮肤类型,家族史,日常行为(如使用护肤品品牌≥4个/d、洗澡/桑拿、使用空调、使用剥脱类洗护用品、运动、洗脸频率>2次/d),医疗行为(如半年内口服维A酸类药物,1个月内外用过壬二酸和维A酸类药物、进行过激光治疗,1周内进行过化学剥脱治疗)和环境因素(如季节交替、粉尘、刮风、夏季、春季、日晒时间4h/d、秋季、地区)可能是痤疮患者继发敏感性皮肤的影响因素。结论本调查初步明确了痤疮患者继发敏感性皮肤的影响因素,提示临床医生在痤疮治疗中应兼顾敏感性皮肤的治疗,以提高痤疮的临床治愈率,降低复发率。Objective To decipher the occurrence and influencing factors of sensitive skin(SS)after acne and to provide guidance for the prevention and treatment of sensitive skin after acne.Methods A cross-sectional survey was conducted on the patients with sensitive skin after acne(n=5787)who participated in the"acne week"in the Dermatology Departments of 76 Medical Centers in China by stratified random sampling.By analyzing demographic information,acne severity,skin type,acne family history,daily behavior,medical behavior,environmental factors,regions and other data,the influencing sensitive skin after acne were identified.Results Among the 5787 cases,3979(68.76%)were female,and the average patient's age was(25.63±6.66)years.There were 1840 cases of sensitive skin secondary to acne,and the detection rate was 31.80%.Logistic regression analysis results showed that female sex,acne severity,skin type,acne family history,daily behaviors,such as the use of skin care products≥4 times per day,bathing/sauna,use of air conditioning,use of stripping products,exercise,wash frequency>2 times per day;medical behaviors such as oral retinoic acid within the previous six months,topical azelaic acid and retinoic acid within the past month,laser treatment,therapeutic chemiculare within the past week;environmental factors such as seasonal alternation,dust,wind,summer,spring,sun time>4 hours per day,autumn,the region may be the influencing factors of secondary sensitive skin in acne patients.Conclusion Our investigation identified the influencing factors of secondary sensitive skin in acne patients,suggesting that clinicians should take sensitive skin into account in treating acne to improve the cure rate of clinical acne and reduce the recurrence rate.
