微螺钉支抗种植体与口外弓强支抗在口腔正畸患者中的应用效果比较

摘  要:目的:比较微螺钉支抗种植体与口外弓强支抗在口腔正畸患者中的应用效果。方法:选取2022年5月年2023年5月该院收治的90例错牙合畸形患者进行前瞻性研究,依据随机数字法将其分为对照组与观察组各45例。对照组采用口外弓强支抗正畸法矫治,观察组采用微螺钉支抗种植体正畸法矫治。比较两组磨牙移位、上中切牙凸距差、上中牙倾角差,治疗前后咀嚼功能、咬合力、龈沟液炎性因子[肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)]水平,以及并发症发生率。结果:观察组磨牙移位、上中切牙凸距差、上中牙倾角差均小于对照组,差异有统计学意义(P<0.05);治疗后,两组咀嚼功能、咬合力均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组TNF-α、hs-CRP、IL-6水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P>0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:微螺钉支抗种植体正畸法矫治口腔正畸患者可提高咀嚼功能和咬合力,以及降低磨牙移位、上中切牙凸距差、上中牙倾角差、龈沟液炎性因子水平和并发症发生率的效果优于口外弓强支抗正畸法矫治。Objective:To compare application effects of micro-screw implant anchorage and headgear anchorage in orthodontic patients.Methods:A prospective study was conducted on 90 patients with malocclusion admitted to the hospital from May 2022 to May 2023.According to the random number method,they were divided into control group and observation group,45 cases in each group.The control group was treated with headgear anchorage orthodontics,while the observation group was treated with micro-screw implant anchorage orthodontics.The molar displacement,the convex distance difference of upper and middle incisors,the inclination angle difference of upper and middle teeth,the masticatory function,the bite force,the levels of inflammatory factors[tumor necrosis factor-α(TNF-α),hyper-sensitive C-reactive protein(hs-CRP),interleukin-6(IL-6)]in gingival crevicular fluid before and after the treatment,and the incidence of complications were compared between the two groups.Results:The molar displacement,the convex distance difference of upper central incisor and the inclination angle difference of upper and middle teeth in the observation group were smaller than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the masticatory function and the bite force of the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of TNF-α,hs-CRP and IL-6 in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Micro-screw implant anchorage orthodontic treatment of the orthodontic patients can improve the masticatory function
