热牙胶垂直加压充填法在急性牙髓炎根管治疗中的应用

摘　　要：目的探讨热牙胶垂直加压充填法在急性牙髓炎根管治疗中的应用效果。方法队列研究。抽取2019年10月至2022年10月于郑州大学第一附属医院拟行根管治疗的80例急性牙髓炎患者为研究对象,采用随机数字表法将其分为对照组和观察组,每组40例。两组均接受一次性根管充填治疗,其中对照组采用冷牙胶侧方加压充填法,观察组采用热牙胶垂直加压充填法。比较两组根管充填质量;于根管治疗后1年,评估治疗效果;记录两组治疗后1年内不良事件发生情况。治疗后3个月、6个月、1年,记录并比较两组患牙咬合力和牙齿松动度。结果观察组恰填率(93.33%,112/120)高于对照组(83.05%,98/118),差异有统计学意义(P<0.05)。观察组治疗有效率(93.10%,54/58)高于对照组(78.57%,44/56),P<0.05。观察组不良事件发生率(3.45%,2/58)低于对照组(16.07%,9/56),P<0.05。治疗后3、6个月,两组牙齿松动度、咬合力比较差异未见统计学意义(P>0.05);治疗后1年,观察组牙齿松动度低于对照组,咬合力高于对照组(P<0.05)。结论热牙胶垂直加压充填法可提高急性牙髓炎根管治疗充填质量、治疗效果及患牙咬合力,降低牙齿松动度,还能降低根管治疗后不良事件发生率。Objective To investigate the application effect of warm vertical compaction of gutta-percha technique in root canal therapy for acute pulpitis.Methods Eighty patients with acute pulpitis who was scheduled for root canal therapy in the First Affiliated Hospital of Zhengzhou University from October 2019 to October 2022 were selected as study subjects for the cohort study.And they were divided into a control group and an observation group by random number table method,with 40 cases in each group.Both groups received one-time root canal filling treatment,the root canal in the control group was filled by lateral compression of cold gum,and root canal in the observation group was filled by warm vertical compaction of gutta-percha technique.The quality of root canal filling of the two groups were compared.One year after root canal treatment,the treatment effects of the two groups were evaluated.The occurrence of adverse events within 1 year after treatment were recorded.The 3-month,6-month,and 1-year postoperative bite force and tooth looseness of the affected teeth were recorded and compared between the two groups.Results The rate of appropriate filling in the observation group(93.33%,112/120)was higher than that in the control group(83.05%,98/118),P<0.05.The treatment effective rate of the observation group(93.10%,54/58)was higher than that of the control group(78.57%,44/56),P<0.05.The incidence of adverse events in the observation group was 3.45%(2/58),lower than the 16.07%(9/56)in the control group(P<0.05).At 3 and 6 months after treatment,there was no statistical significant difference in tooth looseness and bite force between the two groups(P>0.05).One year after treatment,the observation group had lower tooth looseness and higher bite force than the control group(P<0.05).Conclusions The warm vertical compaction of gutta-percha technique can improve the filling quality in root canal treatment for acute pulpitis,enhance the treatment effectiveness,enhance the bite force of affected teeth,reduce tooth looseness,and
