即刻种植修复与残根保存修复在前牙牙体缺损患者中的应用效果比较

摘　　要：目的比较即刻种植修复与残根保存修复在前牙牙体缺损患者中的应用效果。方法队列研究。抽取2022年1月至2023年2月于许昌口腔医院就诊的前牙牙体缺损患者72例,按随机数字表法分为对照组和研究组,每组36例。对照组行残根保存修复,研究组行即刻种植修复。比较两组治疗效果及并发症发生率;比较两组治疗前后牙周指标[龈沟出血指数(SBI)、探诊深度(PD)、菌斑指数(PLI)]、咀嚼效率、红色美学指数(PES)。结果研究组总有效率(94.44%,34/36)高于对照组(72.22%,26/36),P<0.05。治疗后,研究组SBI、PD、PLI低于对照组(P<0.05)。治疗后,研究组咀嚼效率、PES评分高于对照组(P<0.05)。研究组并发症发生率(2.78%,1/36)低于对照组(22.22%,8/36),差异有统计学意义(P<0.05)。结论与残根保存修复相比,即刻种植修复在前牙牙体缺损患者中的应用效果更好,能提升疗效,改善患者牙周指标,提高其咀嚼效率和美学效果,减少修复体脱落、牙龈出血等并发症的发生。Objective To compare the application effects of immediate implant restoration and residual root preservation and repair in the treatment of anterior tooth dental defect.Methods A total of 72 patients with anterior tooth dental defect treated in Xuchang Oral Medicine Hospital from January 2022 to February 2023 were selected for the cohort study.And they were divided into the control group and the study group according to the random number table method,with 36 cases in each group.The control group was treated by residual root preservation and repair,while the study group was treated by immediate implant restoration.The treatment effect and incidence of complications were compared between the two groups.The preoperative and postoperative periodontal indices,including sulcus bleeding index(SBI),probing depth(PD)and plaque index(PLI),chewing efficiency,and pink esthetic scores(PES)were compared between the two groups.Results The total effective rate of the study group(94.44%,34/36)was higher than that of the control group(72.22%,26/36),P<0.05.After treatment,the study group had lower SBI,PD,and PLI compared with the control group(P<0.05).Compared with the control group,the chewing efficiency and PES score of the study group were higher after treatment(P<0.05).The incidence of complications of the study group was 2.78%(1/36),lower than the 22.22%(8/36)of the control group(P<0.05).Conclusions Compared with residual root preservation and repair,immediate implant restoration has a better effect in the treatment of anterior tooth dental defect.It can improve the curative effect,improve periodontal indices,enhance chewing efficiency and aesthetic effect,and reduce the incidence of complications,such as prosthesis shedding and gum bleeding.
