摘　　要：脊柱骨巨细胞瘤是一种交界性的原发性骨肿瘤,约占所有原发性骨肿瘤的2%~15%。脊柱骨巨细胞瘤的治疗目标是切除肿瘤病灶、缓解疼痛、重建脊柱稳定性和改善神经功能,手术治疗是最主要的治疗方式,包括肿瘤整块切除、次全切除和肿瘤刮除。脊柱骨巨细胞瘤手术难度大、术后复发率高,且复发后的治疗难度加大,再手术的彻底切除率低。随着脊柱肿瘤外科治疗理念的更新、技术的进步、靶向药物的问世和多学科综合治疗的日益完善,脊柱骨巨细瘤的治愈率显著提高,但迄今为止脊柱骨巨细胞瘤诊疗尚无统一的规范和指导原则。中国医师协会骨科医师分会脊柱肿瘤学组和中国医院协会临床新技术应用专业委员会组织国内相关领域的专家,结合国内脊柱骨巨细胞瘤现状和专家经验制订了《脊柱骨巨细胞瘤诊疗中国专家共识》。共识旨在规范脊柱骨巨细胞瘤的诊疗原则和流程,从而改善脊柱骨巨细胞瘤患者的预后。Giant cell tumor(GCT)of the spine is a borderline primary bone tumor,accounting for about 2%to 15%of all primary bone tumors.The treatment goals for spinal GCT are to remove the tumor lesion,relieve pain,rebuild spinal stability,and improve neurological function.Surgical treatment is the most important method,including en bloc resection,subtotal resection and tumor curettage.However,spinal GCT surgery is difficult,with a high rate of postoperative recurrence.Treatment of recurrent cas-es is more challenging,with a lower rate of complete resection in revision surgeries.With the update of the concept of surgical treatment of spinal tumors,the progress of technology,the advent of targeted drugs and the improvement of multidisciplinary com-prehensive treatment,the cure rate of spinal GCT has been significantly improved.However,there are still no unified standards or guidelines for diagnosing and treating spinal GCT.Spinal Tumors Study Group of the Chinese Association of Orthopaedic Surgeons and the Chinese Hospital Association Clinical Medical Technology Application Committee organized domestic experts in related fields to formulate the"Chinese expert consensus on the diagnosis and treatment of giant cell tumor of the spine"based on the cur-rent status and expert experience of GCT of the spine in China.The purpose of this consensus is to standardize the principles and procedures of diagnosis and treatment of spinal GCT,so as to improve the prognosis of patients with spinal GCT.
