内镜下钛夹止血联合泮托拉唑、生长抑素治疗上消化道大出血的效果及对凝血指标的影响

摘　　要：目的探究内镜下钛夹止血联合泮托拉唑、生长抑素治疗上消化道大出血的效果及对凝血指标的影响。方法择取2021年1月至2022年12月收治的120例上消化道大出血患者为研究对象,随机将其分为对照组和观察组,每组60例。对照组采用泮托拉唑、生长抑素治疗,观察组在对照组基础上加内镜下钛夹止血治疗。比较两组的治疗效果。结果观察组的治疗总有效率为95.00%,高于对照组的80.00%(P<0.05)。治疗后,观察组的凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)短于对照组,纤维蛋白原(FIB)水平及血小板计数(PLT)高于对照组(P<0.05)。治疗后,观察组的胃泌素(GAS)、胃动素(MTL)水平低于对照组,生长抑素(SS)水平高于对照组(P<0.05)。结论内镜下钛夹止血联合泮托拉唑、生长抑素治疗上消化道大出血的效果明显,不仅能够改善凝血指标,还能调节胃肠激素水平,减轻出血症状。Objective To investigate the effect of endoscopic titanium clip hemostasis combined with pantoprazole and somatostatin in the treatment of upper gastrointestinal hemorrhage and its influences on coagulation indexes.Methods A total of 120 patients with upper gastrointestinal hemorrhage admitted from January 2021 to December 2022 were selected as study objects and randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with pantoprazole and somatostatin,and the observation group was treated with endoscopic titanium clip hemostasis on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 95.00%,which was higher than 80.00%in the control group(P<0.05).After treatment,the prothrombin time(PT)and activated partial thromboplastin time(APTT)in the observation group were shorter than those in the control group,and the fibrinogen(FIB)level and platelet count(PLT)were higher than those in the control group(P<0.05).After treatment,the levels of gastrin(GAS)and motilin(MTL)in the observation group were lower than those in the control group,and the level of somatostatin(SS)was higher than that in the control group(P<0.05).Conclusion Endoscopic titanium clip hemostasis combined with pantoprazole and somatostatin in the treatment of upper gastrointestinal hemorrhage has effective effect.It can not only improve the coagulation index,but also regulate the level of gastrointestinal hormones and reduce the symptoms of bleeding.
