基于“毒瘀”理论探讨缺血性卒中的病机证治

摘  要:缺血性卒中(IS)病机复杂,变证常起,多遗留偏瘫、失语等神经系统后遗症,属于临床危重、难治病症。纵古贯今,众多医家认为瘀阻脑脉为IS的基本病机,而“脑脉瘀久,瘀积毒化”形成瘀毒交织之局势,毒瘀叠加为患,引起脑髓神机受损甚或损伤加重,是以提出“毒瘀”是IS病重难治的重要病理机转。本文基于“毒瘀”理论分析了IS的病机证治,并指出瘀阻致栓为其基本病机,瘀积化毒为其病理机转,瘀毒致炎为其病损关键,瘀毒伤髓为其病理转归,这可以为临床防治IS提供思路。Ischemic stroke has complex pathogenesis,frequent changes,and neurological sequelae such as hemiplegia and aphasia.It is one of the clinical critical and refractory diseases.Throughout history,many physicians have considered that blood stasis in the brain vessels has the fundamental pathogenesis of ischemic stroke(IS).The concept that"prolonged stasis in the brain vessels leads to the accumulation and transformation into toxins"results in a complex interplay of stasis and toxins.The accumulation of toxins and stasis exacerbates the condition,causing damage oReven worsening of the brain's vital functions.Therefore,the"toxic-stasis"is proposed as an important pathogenesis transformation foRsevere and refractory IS.Based on the"toxic-stasis"theory,this article analyzes the pathogenesis and treatment of IS,and points out that stasis leading to thrombosis is the basic pathogenesis of IS,the stagnation and accumulation transforming into toxin is the pathogenesis transformation,the stasistoxin inducing inflammation is the key to damage disease of IS,and the stasis-toxin damaging the marrow is the pathological outcome of IS.This can provide insights foRthe clinical prevention and treatment of IS.
