血清PGⅠ/PGⅡ、CYFRA21-1、CA125对胃癌的诊断价值

摘　　要：目的探究血清胃蛋白酶原Ⅰ(PGⅠ)/胃蛋白酶原Ⅱ(PGⅡ)、细胞角蛋白19片段(CYFRA21-1)、糖类抗原125(CA125)对胃癌诊断价值。方法选取200例胃部病变患者,经胃镜及手术病理活检确诊,94例早期胃癌(胃癌组),71例慢性非萎缩性胃炎和35例慢性萎缩性胃炎(良性病变组),比较两组及胃癌组不同临床病理特征患者血清PGⅠ/PGⅡ、CYFRA21-1、CA125,分析血清PGⅠ/PGⅡ、CYFRA21-1、CA125与临床病理特征的相关性及对胃癌诊断效能。结果胃癌组血清PGⅠ/PGⅡ低于良性病变组,CYFRA21-1、CA125高于良性病变组,差异有统计学意义(P<0.05)。CYFRA21-1、CA125联合诊断胃癌的ROC下面积(AUC)大于单独血清指标检测(P<0.05)。与中高分化程度、TNM分期Ⅰ~Ⅱ期、无淋巴结转移患者比较,低分化程度、TNM分期Ⅲ~Ⅳ期、淋巴结转移患者血清PGⅠ/PGⅡ、CYFRA21-1、CA125水平较高(P<0.05)。血清CYFRA21-1、CA125、PGⅠ/PGⅡ与TNM分期、淋巴结转移、分化程度显著相关(P<0.05)。结论血清PGⅠ/PGⅡ、CYFRA21-1、CA125表达均与胃癌临床病理特征有关,联合检测可作为诊断胃癌的标志物。Objective To investigate the diagnostic value of serum pepsinogenⅠ(PGⅠ)/pepsinogenⅡ(PGⅡ),cytokeratin 19 fragment(CYFRA21-1)and carbohydrate antigen 125(CA125)in gastric cancer.Methods 200 patients with gastric lesions were selected.94 cases were confirmed as early gastric cancer(gastric cancer group),71 cases were confirmed as chronic non-atrophic gastritis,and 35 cases were confirmed as chronic atrophic gastritis(benign disease group)by gastroscopy and surgical pathological biopsy.Serum levels of PGⅠ/PGⅡ,CYFRA21-1,and CA125 were compared between the two groups and within the gastric cancer group with varying clinicopathological characteristics.Furthermore,correlations between these markers,clinicopathological characteristics,and diagnostic efficacy for gastric cancer were analyzed.Results Compared with benign lesion group,serum PGⅠ/PGⅡin gastric cancer group was lower,and CYFRA21-1 and CA125 were higher,with a statistically significant difference(P<0.05).The Area under the ROC(AUC)of PGⅠ/PGⅡ,CYFRA21-1 and CA125 combined with gastric cancer was greater than that of single serum indexes(P<0.05).Serum levels of PGⅠ/PGⅡ,CYFRA21-1 and CA125 were higher in patients with low differentiation,TNM stagesⅢtoⅣand lymph node metastasis than in patients with middle and high differentiation,TNM stagesⅠtoⅡand no lymph node metastasis(P<0.05).Serum CYFRA21-1,CA125 and PGⅠ/PGⅡwere significantly correlated with TNM stage,lymph node metastasis and differentiation degree(P<0.05).Conclusion The expressions of serum PGⅠ/PGⅡ,CYFRA21-1 and CA125 are related to the clinicopathological features of gastric cancer,and the combined detection can be used as markers for the diagnosis of gastric cancer.
