肛管癌患者早期死亡的影响因素:一项基于SEER数据库的回顾性研究

摘　　要：目的:探讨肛管癌患者早期死亡的影响因素。方法:从美国监测、流行病学和结果(SEER)数据库中筛选6852例肛管癌患者。将初始诊断后3个月内死亡的肛管癌患者定义为肛管癌早期死亡。根据单因素及多因素Logistic回归探索肛管癌患者早期死亡的影响因素。结果:单因素Logistic回归显示,年龄、参保情况、婚姻状况、病理分级、分期、骨转移、脑转移、肺转移、肝转移、手术、放化疗是肛管癌患者早期死亡的影响因素,差异有统计学意义(P<0.05)。多因素Logistic回归结果显示,年龄、婚姻状态、分期、骨转移、肝转移、手术、化疗是肛管癌患者早期死亡的独立影响因素,差异有统计学意义(P<0.05)。结论:年龄、婚姻状态、分期、骨转移、肝转移、手术、化疗是肛管癌患者的早期死亡的影响因素,应根据影响预后的因素对患者进行个体化干预。Objective To explore the influencing factors of early death in patients with anal cancer.Method 6852 patients with anal canal cancer were screened from the SEER database.Patients with anal cancer who die within three months of initial diagnosis were defined as early deaths from anal cancer.Exploring the influencing factors of early mortality in anal cancer patients based on univariate and multivariate logistic regression.Results Univariate logistic regression showed that age,insurance status,marital status,pathological grading,staging,bone metastasis,brain metastasis,lung metastasis,liver metastasis,surgery,and radiotherapy and chemotherapy were the influencing factors of early death in anal cancer patients(P<0.05).The results of multiple logistic regression showed that age,marital status,stage,bone metastasis,liver metastasis,surgery,and chemotherapy were independent influencing factors for early mortality in anal cancer patients(P<0.05).Conclusion This study showed that age,marital status,stage,bone metastasis,liver metastasis,surgery,and chemotherapy are influencing factors for early death in patients with anal cancer.Individualized interventions should be carried out based on the factors that affect prognosis.
