摘 要:目的探究磁共振(MR)影像组学相关指标与超声定量参数对乳腺癌患者新辅助化疗(NACT)疗效的预测价值。方法收集98例乳腺癌患者临床资料。所有患者均进行NACT治疗,并实施MR及超声检测。依照患者NACT疗效分为良好组(77例)和不良组(21例)。比较2组年龄、体质指数、病理学类型、分子分型、肿瘤分期、组织分级;比较2组MR影像组学相关指标、超声定量参数;受试者操作特征曲线(ROC)下面积(AUC)评价MR影像组学相关指标与超声定量参数对乳腺癌患者NACT疗效的预测效能。结果2组年龄、体质指数、病理学类型、分子分型、肿瘤分期、组织分级比较,差异无统计学意义(P>0.05)。2组MR影像组学相关指标FGT、肿瘤边缘、肿瘤最大径比较,差异无统计学意义(P>0.05);2组BPE、肿瘤形状比较,差异有统计学意义(t=4.633,P=0.031、t=4.455,P=0.035)。2组超声参数△D、△TTP、△Vmean比较,差异无统计学意义(P>0.05);2组△PE、△Vmax比较,差异有统计学意义,且良好组显著高于不良组(t=3.085,P=0.003、t=3.864,P=0.000)。MR影像组学相关指标BPE、肿瘤形状与超声定量参数△PE、△Vmax联合预测乳腺癌患者NACT疗效的价值较高,四项指标联合的AUC高于单一指标,灵敏度、特异度相对单一指标较好。结论MR影像组学相关指标与超声定量参数对乳腺癌患者NACT疗效有良好的预测价值。Objective To explore the predictive value of magnetic resonance(MR)imaging related indexes and ultrasound quantitative parameters for the efficacy of neoadjuvant chemotherapy(NACT)in breast cancer patients.Methods The clinical data of 98 patients with breast cancer were collected.All patients were treated with NACT,and MR And ultrasound were performed.Patients were divided into good group(77 cases)and bad group(21 cases)according to NACT efficacy.Age,body mass index,pathological type,molecular type,tumor stage and tissue grade were compared between the 2 groups.MR Imaging related indexes and ultrasonic quantitative parameters were compared between the 2 groups.The area under receiver operating characteristic curve(ROC)(AUC)evaluated the predictive efficacy of MR Imaging related indicators and ultrasound quantitative parameters on NACT efficacy in breast cancer patients.Results There was no statistical significance in age,body mass index,pathological type,molecular type,tumor stage and tissue grade between the two groups(P>0.05).There was no significant difference in MR Imaging related indexes FGT,tumor margin and maximum diameter between the 2 groups(P>0.05).There were significant differences in BPE and tumor shape between the 2 groups(t=4.633,P=0.031,t=4.455,P=0.035).There was no significant difference in ultrasonic parameters△D,△TTP,△Vmean between the 2 groups(P>0.05).The difference of△PE and△Vmax between the two groups was statistically significant,and the good group was significantly higher than the bad group(t=3.085,P=0.003,t=3.864,P=0.000).The combined value of MR Imaging related indicators BPE,tumor shape and ultrasonic quantitative parameters△PE and△Vmax in predicting the efficacy of NACT in breast cancer patients is higher.The combined AUC of the four indicators was higher than that of a single indicator,and the sensitivity and specificity are better than that of a single indicator.Conclusion MR Imaging index and ultrasound quantitative parameters have good predictive value for NACT efficac
