摘 要:目的探讨电子计算机断层扫描(CT)联合磁共振成像(MRI)在原发性脊柱骨肿瘤良恶性鉴别诊断中的应用价值。方法选取67例原发性脊柱骨肿瘤患者,均行CT与MRI检查,以病理学检查为“金标准”,比较CT、MRI及二者联合对原发性脊柱骨肿瘤良恶性的诊断准确性。结果病理学检查显示,恶性骨肿瘤31例(46.27%),良性骨肿瘤36例(53.73%)。CT诊断原发性恶性脊柱骨肿瘤敏感度67.74%,特异度75.00%,准确性71.64%,Kappa值0.43;MRI诊断原发性恶性脊柱骨肿瘤敏感度80.65%,特异度77.78%,准确性79.10%,Kappa值0.58;CT联合MRI诊断原发性恶性脊柱骨肿瘤敏感度93.55%,特异度94.44%,准确性94.03%,Kappa值0.88;CT联合MRI诊断原发性恶性脊柱骨肿瘤敏感度明显高于CT(P<0.05),诊断特异度、准确性明显高于CT、MRI(P<0.05)。结论CT联合MRI能够更准确诊断原发性脊柱骨肿瘤的性质,可为临床诊治提供可靠依据。Objective To investigate the application value of computerized tomography(CT)combined with magnetic resonance imaging(MRI)in the differential diagnosis of benign and malignant primary spinal bone tumors.Methods A total of 67 patients with primary spinal bone tumors were selected.All received CT and MRI examinations.Pathological examination was taken as the golden standard to compare the diagnostic accuracy of CT,MRI,and their combination for benign and malignant primary spinal bone tumors.Results Pathological examination showed 31(46.27%)cases of malignant bone tumors and 36(53.73%)cases of benign bone tumors.The sensitivity,specificity,accuracy and Kappa value of CT for diagnosing malignant primary spinal bone tumors were 67.74%,75.00%,71.64%and 0.43.The sensitivity,specificity,accuracy and Kappa value of MRI for diagnosing malignant primary spinal bone tumors were 80.65%,77.78%,79.10%and 0.58.The sensitivity,specificity,accuracy and Kappa value of CT combined with MRI for diagnosing malignant primary spinal bone tumors were 93.55%,94.44%,94.03%and 0.88.The sensitivity of CT combined with MRI for diagnosing malignant primary spinal bone tumors was significantly higher than that of CT(P<0.05).The diagnostic specificity and accuracy were significantly higher than those of CT or MRI(P<0.05).Conclusion CT combined with MRI is more accurate in diagnosing the nature of primary spinal bone tumors,providing a reliable basis for clinical diagnosis and treatment.
