气血双补法联合运动疗法对癌因性疲乏的临床疗效研究

摘　　要：目的探讨气血双补法联合运动疗法治疗癌因性疲乏的效果。方法选取癌症患者60例为研究对象,采用随机数字表法将患者分为对照组(30例)和观察组(30例)。对照组采取止痛、止呕、改善睡眠、调节免疫力、心理辅导等方法进行症状管理,并鼓励患者积极运动,观察组在采取上述对症治疗和鼓励积极运动的同时给予八珍汤加味内服。比较两组患者治疗前及治疗后T细胞亚群(CD3、CD4、CD8、CD4/CD8);不良反应发生情况;生活质量评分;疲乏评分及气血两虚症候评分。结果观察组中检测的PFS-R评分与气血两虚证辩证评分治疗后均低于治疗前,具有统计学差异(P<0.05);观察组中PFS-R评分与气血两虚证辩证评分治疗后明显优于对照组,具有统计学差异(P<0.05)。治疗后观察组中CD3+、CD4+、CD4+/CD8+水平较对照组与治疗前升高,具有统计学差异(P<0.05);治疗后观察组中CD8+水平较对照组与治疗前下降,具有统计学差异(P<0.05)。观察组中治疗后KPS评分较对照组与治疗前升高,具有统计学差异(P<0.05)。观察组白细胞减少、恶心呕吐、红细胞减少发生率均低于对照组,差异具有统计学意义(P<0.05)。结论根据癌因性疲乏患者中医证候特点运用八珍汤联合运动疗法进行治疗,可以降低患者疲乏评分,增强患者免疫功能,改善生存质量,降低治疗费用提供新的中医治疗模式。Objective To investigate the effect of Qi-blood supplement combined with exercise therapy on cancer-related fatigue.Methods 60 patients with malignant were selected as the study objects.The patients were divided into the control group(30 cases)and the observation group(30 cases)by random number table method.The control group was treated with pain relief,antiemesis,immune regulation,sleep improvement,nutritional support,psychological care and exercise,while the observation group took the above symptomatic treatment and encouraged active exercise at the same time given Bazhen decoction with added flavor.Fatigue score,Qi-blood deficiency syndrome score,T lymphocyte subsets including(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)),quality of life scores and adverse reactions were compared between the 2 groups.Results PFS-R score and Qi-blood deficiency syndrome dialectical score in the observation group were lower after treatment than before treatment,with statistical difference(P<0.05).PFS-R score and Qi-blood deficiency syndrome dialectical score in the observation group were significantly better than those in the control group after treatment,with statistical difference(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the observation group were higher than those before treatment,with statistical difference(P<0.05);The level of CD8^(+)in the observation group decreased after treatment compareing with control group before treatment,with statistical difference(P<0.05).The KPS score in the observation group after treatment was higher than that before treatment,with statistical difference(P<0.05).The incidence of leukopenia,nausea,vomiting and erythropenia in the observation group was lower than that in the control group,with statistical significance(P<0.05).Conclusion According to the characteristics of traditional Chinese medicine syndromes of patients with cancer-related fatigue,the application of Bazhen decoction combined with exercise therapy can reduce the fatigue score of patients,enhance the imm
