内镜下光动力疗法联合氟尿嘧啶与奥沙利铂治疗晚期贲门—胃底癌患者的临床研究

摘　　要：目的:探讨内镜下光动力疗法(PDT)联合氟尿嘧啶与奥沙利铂治疗晚期贲门—胃底癌患者的临床效果。方法:回顾性分析2020年1月—2022年1月安阳市中医院收治的82例晚期贲门—胃底癌症患者的临床资料,根据治疗方法的不同将其分为对照组和观察组,每组各41例。对照组患者接受氟尿嘧啶与联合奥沙利铂治疗,观察组患者在对照组的基础上接受内镜下PDT治疗。比较两组患者血清肿瘤标志物水平、治疗效果、并发症发生情况及生活质量。结果:治疗后,两组患者MMP-2、CA125、CEA水平均低于治疗前,且观察组患者低于对照组患者,差异均有统计学意义(t=2.215、2.687、4.337,P<0.05)。两组患者治疗总有效率比较,差异无统计学意义(χ^(2)=4.556,P>0.05)。两组患者不良反应发生率比较,差异无统计学意义(χ^(2)=0.082,P>0.05)。观察组患者治疗8周后、治疗12周后生活质量评分均高于对照组患者,差异均有统计学意义(t=3.464、4.152,P<0.05)。结论:内镜下PDT联合氟尿嘧啶与奥沙利铂治疗晚期贲门—胃底癌患者,能够提高其血清肿瘤标志物水平,改善其治疗效果,提高其生活质量,且安全性较高。Objective:To study the clinical effect of endoscopic photodynamic therapy(PDT)combined with fluorouracil and oxaliplatin in the treatment of advanced cardia fundus cancer.Methods:A retrospective analysis was conducted on the clinical data of 82 patients with advanced cardia fundus cancer admitted to the hospital from January 2020 to January 2022.According to the treatment method,they were divided into two groups,with 41 cases in each group.The control group were treated with fluorouracil and combined with oxaliplatin.The observation group were treated with endoscopic PDT on the basis of the control group.The levels of serum tumor markers,therapeutic effects,complications and quality of life scores were analyzed before and after treatment.Results:After treatment,the levels of serum matrix metalloproteinase-2(MMP-2),carbohydrate antigen 125(CA125)and carcinoembryonic antigen(CEA)in the observation group were significantly lower than those in the control group(t=2.215,2.687,4.337;P<0.05).There was no significant difference of the total effective rate between the two groups(χ^(2)=4.556,P>0.05).There was no significant difference of adverse effect rates between the two groups(χ^(2)=0.082,P>0.05).The quality of life score of the observation group 8 and 12 weeks after treatment were higher than the control group(t=3.464,4.152;P<0.05).Conclusion:Endoscopic PDT combined with fluorouracil can improve the level of serum tumor markers,the therapeutic effect and the quality of life in patients with advanced cardiac fundus cancer,and it is safe.
