高频超声联合超声造影检查对甲状腺乳头状癌检出率的影响

摘　　要：目的:探讨高频超声(HFUS)联合超声造影(CEUS)检查对甲状腺乳头状癌检出率的影响,以期为临床早期筛查诊断、制定治疗方案提供参考。方法:选取2020年8月—2022年8月漯河市中医院收治的98例疑似甲状腺癌患者作为研究对象,入院后均行HFUS、CEUS检查,以手术或病理活检结果为金标准,统计比较HFUS、CEUS单独检查及联合诊断结果和诊断效能,并比较HFUS、CEUS单独检查及联合诊断对于不同类型甲状腺癌及淋巴结转移的检出率。结果:与HFUS、CEUS检查单独诊断比较,联合诊断灵敏度、准确度较高,漏诊率较低,差异有统计学意义(χ^(2)=7.525、6.353、7.525,P<0.05);HFUS、CEUS联合诊断甲状腺乳头状癌阳性检出率及对于甲状腺癌颈部淋巴结皮质增加、淋巴门偏移、淋巴结融合检出率均高于单一检测,差异有统计学意义(χ^(2)=8.444、7.486、10.015、7.750,P<0.05)。结论:HFUS、CEUS联合诊断可显著提高甲状腺乳头状癌阳性检出率、诊断灵敏度,降低漏诊率,有助于临床治疗方案的选择,具有较高的临床应用价值。Objective:To investigate the effect of high-frequency ultrasound(HFUS)combined with contrastus-enhanced ultrasound(CEUS)on the detection rate of papillary thyroid carcinoma,in order to provide reference for early clinical screening and diagnosis and treatment.Methods:A total of 98 patients with suspected thyroid cancer admitted to the hospital from August 2020 to August 2022 were selected,and HFUS and CEUS examinations were performed after admission.The results of surgery or pathological biopsy were used as the gold standard.The diagnostic results and diagnostic efficacy of HFUS and CEUS examinations were statistically compared.The detection rates of HFUS,CEUS alone and combined diagnosis for different types of thyroid cancer and lymph node metastasis were compared.Results:Compared with the single diagnosis of HFUS and CEUS,the sensitivity and accuracy of combined diagnosis were higher,and the rate of missed diagnosis was lower(χ^(2)=7.525,6.353,7.525;P<0.05).The positive rate of HFUS and CEUS combined diagnosis of thyroid papillary carcinoma and the positive rate of cervical lymph node cortex increase,lymphatic portal deviation and lymph node fusion were higher than that of single detection,and the difference was significant(χ^(2)=8.444,7.486,10.015,7.750;P<0.05).Conclusion:The combined diagnosis of HFUS and CEUS can significantly improve the positive detection rate and diagnostic sensitivity of papillary thyroid carcinoma,reduce the rate of missed diagnosis,and contribute to the selection of clinical treatment,which has high clinical application value.
