牙线牵引辅助内镜黏膜下剥离术治疗胃黏膜下肿瘤的效果分析

摘　　要：目的探讨牙线牵引辅助内镜黏膜下剥离术治疗胃黏膜下肿瘤的效果。方法选取2020年6月—2022年6月本院收治的100例胃间质瘤患者为研究对象,以随机法将其分为对照组和试验组,每组50例。对照组采用金属夹弹力圈联合牵引辅助内镜黏膜下剥离术,试验组采用牙线牵引辅助内镜黏膜下剥离术。比较两组手术的各项指标、并发症发生率、肿瘤复发和转移率。结果两组在出血量、暴露瘤体用时、切除病灶用时、手术用时方面的差异不明显(P>0.05);两组的并发症发生率无显著差异(P>0.05);两组观察期间内的复发率和转移率无显著差异(P>0.05)。结论牙线牵引辅助技术在胃间质瘤黏膜下剥离术中有一定的应用价值,临床上需要根据患者的具体情况选择合适的牵引技术。Objective To explore application value of floss traction assisted technology in submucosal dissection of gastric stromal tumors.Methods The paper chose 100 samples of gastric stromal tumors enrolled and treated in Shenzhen City Bao'an District Central Hospital from June 2020 to June 2022,and divided them into control group and experimental group randomly,with 50 cases in each group.Control group was treated with metal clip elastic ring combined with traction assisted endoscopic submucosal dissection,while experimental group with floss traction assisted endoscopic submucosal dissection.Surgical indicators,incidence of complications,tumor recurrence and metastasis rates were compared between two groups.Results Bleeding volume of experimental group was 14.52±1.60(mL),exposure time of the tumor body was 35.74±11.57(min),resection time of the lesion was 37.25±8.14(min),and surgical time was 76.31±10.24(min).There was no significant difference compared with control group,P>0.05.There was one cases with complication of bleeding in experimental group,and two cases with complications of bleeding and perforation respectively in control group.Comparison between groups showed P>0.05.There was no recurrence or metastasis in experimental group during observation period,there was one cases of recurrence in control group,P>0.05.Conclusion Floss traction assisted technology can achieve some certain application value in submucosal dissection of gastric stromal tumors.During clinical practice,it is necessary to choose appropriate traction technology based on specific situation of patients.
