摘  要:目的:探究多回波Dixon技术定量分析高BMI人群脂肪肝含量及治疗效果。方法:选取2023年1月—2024年6月在萍乡市人民医院检查的高BMI成人90例,参考非酒精性脂肪性肝病诊疗指南将高BMI肥胖人群分为3组,肥胖A组、肥胖B组、肥胖C组,每组各30例。所有患者均行MRI多回波Dixon序列扫描,并收集所有患者临床资料,统计多回波Dixon技术诊断脂肪肝的准确率和灵敏度,观察脂肪分数与形态指标及实验室指标的相关性,对比分析治疗前、后肝脏脂肪含量的变化。结果:Spearman相关分析,FF与BMI、腰围、ALT、AST、低密度脂蛋白、甘油三酯呈正相关性(P<0.01),与高密度脂蛋白呈负相关性(P<0.01);三组患者的FF值对比存在差异性(P<0.001),且随着病情的加重患者的FF值增高;ALT的AUC(曲线下面积)为0.677,敏感度为0.437,特异度为1,95%置信区间为0.566~0.788,P为0.018,具有统计学意义(P<0.05)。AST的AUC为0.702,敏感度为0.451,特异度同样为1,95%置信区间为0.584~0.819,P为0.007,显示出更强的统计学显著性(P<0.01);治疗后患者(6.17±3.85)%,明显低于治疗前患者的FF值(13.41±9.35)%,差异具有统计学意义(t=10.037,P<0.01)。肥胖A组、B组和C组在多项代谢指标上存在显著差异。肥胖C组的治疗前后FF值、BMI、腰围、低密度脂蛋白(LDL)、ALT、AST、甘油三酯、空腹血糖等指标均显著高于A组和B组,显示出肥胖C组的代谢异常最为严重。肥胖B组的上述指标也高于A组,但整体情况优于C组。同时,肥胖A组的高密度脂蛋白(HDL)水平明显高于B组和C组。结论:多回波Dixon技术可在一定程度上对高BMI人群肝脏脂肪情况进行定量测定,可作为肝脏脂肪动态监测、疗效评估和随访观察的有效手段。Objective:To explore the quantitative analysis of fatty liver content in high BMI population and its therapeutic effect by multi-echo Dixon technique.Methods:Atotal of 90 adults with high BMI who were examined in Pingxiang People's Hospital from January 2023 to June 2024 were divided into 3 groups,obese group A,obese group B and obese group C,with 30 cases in each group,according to the diagnosis and treatment guidelines for non-alcoholic fatty liver disease.MRI multi-echo Dixon sequence scan was performed in all patients,and clinical data of all patients were collected.The accuracy and sensitivity of multi-echo Dixon technique in the diagnosis of fatty liver were statistically analyzed,and the correlation between fat fraction and morphological indexes and laboratory indexes was observed,and the changes of liver fat content before and after treatment were compared.Results:Spearman correlation analysis showed that FF was positively correlated with BMI,waist circumference,ALT,AST,low density lipoprotein and triglyceride(P<0.01),but negatively correlated with high density lipoprotein(P<0.01).The FF value of the three groups was different(P<0.001),and the FF value increased with the aggravation of the disease.The AUC(area under the curve)of ALT was 0.677,the sensitivity was 0.437,the specificity was 1,and the 95%confidence interval was 0.566~0.788,with a P value of 0.018,which was statistically significant(P<0.05).The AUC of AST was 0.702,the sensitivity was 0.451,and the specificity was also 1,95%confidence interval was 0.584~0.819,P value was 0.007,indicating a stronger statistical significance(P<0.01).The FF value of patients after treatment(6.17±3.85)%was significantly lower than that of patients before treatment(13.41±9.35)%,and the difference was statistically significant(t=10.037,P<0.01).Obesity group A,B and C have significant differences in multiple metabolic indexes.FF value,BMI,waist circumference,low-density lipoprotein(LDL),ALT,AST,triglyceride and fasting blood glucose before and after treatment in ob
