摘　　要：目的分析宫颈液基细胞学检查(TCT)、人乳头瘤病毒(HPV)检测与宫颈活检组织病理学诊断的关系。方法选取2020年1月—2023年6月本院收治的100例宫颈病变患者为研究对象,所有患者均行TCT检查、HPV检测及宫颈活检组织病理学检查,以病理学检查为金标准,评价TCT检查、HPV检测、TCT检查联合HPV检测的诊断价值。结果100例患者经宫颈活检组织病理学检查确诊为宫颈炎症30例、宫颈上皮内瘤变(CIN)Ⅰ20例、CINⅡ18例、CINⅢ14例、宫颈癌18例;TCT检查结果为非典型鳞状上皮细胞(ASC-US)37例、低度鳞状上皮内病变(LSIL)23例、高度鳞状上皮内病变(HSIL)22例、宫颈癌18例;HPV检测结果为阴性40例,阳性60例;TCT检查联合HPV检测结果为阴性31例,阳性69例。TCT检查联合HPV检测的敏感性、特异性与准确性明显高于单独进行TCT检查和HPV检测(P<0.05)。结论TCT检查联合HPV检测能够提高临床诊断的准确率,可在宫颈病变筛查中推广使用。Objective To analyze relationship between cervical based thinprep cytologic(TCT)test,human papillomavirus(HPV)detection,and pathological diagnosis of cervical biopsy tissue.Methods The paper chose 100 patients with cervical lesions in gynecology department of the hospital from January 2020 to June 2023,treated with TCT examination HPV examination and cervical biopsy tissue pathological examination.Diagnostic value of TCT examination,HPV detection,TCT combined with HPV examination was evaluated with gold standard of pathological examination.Results 30 cases were diagnosed with cervicitis by pathological examination of cervical biopsy tissue,20 cases with CIN I,18 cases of CINⅡ,14 cases of CINⅢand 18 cases of cervical cancer among 100 cases.TCT examination results showed,there were 37 cases of ASC-US,23 cases of LSILs,22 cases of HSIL and 18 cases of cervical cancer.HPV test results showed,there were 40 cases negative and 60 cases positive,combination of TCT and HPV test showed,there were 31 cases negative and 69 cases positive.Sensitivity,specificity and accuracy of TCT combined with HPV examination were significantly higher than TCT examination and HPV examination only,with statistical significance(P<0.05).Conclusion Combination of TCT and HPV examination can improve accuracy of clinical diagnosis,which can be widely applied in cervical lesion screening.
