超声乳化摘除白内障治疗葡萄膜炎继发性青光眼的疗效分析

摘　　要：目的 观察超声乳化摘除白内障治疗葡萄膜炎继发性青光眼的疗效。方法 选取2022年1—12月本院接诊的葡萄膜炎继发性青光眼伴白内障患者84例为研究对象,并按照随机数字表法分为对照组和观察组,每组42例。其中,对照组使用常规白内障囊外摘除治疗,观察组使用超声乳化摘除白内障治疗。观察两组眼压和视力情况、角膜内皮细胞相关指标情况、临床疗效及并发症情况。结果 治疗前,两组患者视力、眼压比较,差异无统计学意义(P>0.05);治疗后,观察组眼压低于对照组,观察组视力高于对照组,差异有统计学意义(P<0.05)。治疗前,两组角膜中心厚度、平均细胞面积、六角形细胞比率、细胞面积变异系数、角膜内皮细胞密度比较,差异无统计学意义(P>0.05);治疗后,观察组角膜中心厚度、平均细胞面积、六角形细胞比率、细胞面积变异系数高于对照组,观察组角膜内皮细胞密度低于对照组,差异有统计学意义(P<0.05)。观察组治疗有效率高于对照组,差异无统计学意义(P>0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 葡萄膜炎继发性青光眼伴白内障患者采用超声乳化摘除白内障治疗效果理想,可以明显改善患者眼压、视力、角膜内皮细胞情况,临床疗效理想,并发症发生率低,可推广应用。Objective To observe curative effect of phacoemulsification for cataract removal in treatment of secondary glaucoma caused by uveitis.Methods The paper chose 84 secondary glaucoma patients with cataracts caused by uveitis in our hospital from January 2022 to December 2022 as study subjects,and divided them into two groups with random number table methods.42 cases in control group were treated with routine extracapsular cataract removal,and 42 cases in observation group with phacoemulsification for cataract removal.Intraocular pressure,visual acuity,corneal endothelial cell related indicators,clinical effect,and complications were observed between two groups.Results Before treatment,there was no difference in visual acuity and intraocular pressure between two groups(P>0.05).After treatment,intraocular pressure in observation group was lower than control group,visual acuity in observation group was higher than control group,with significant difference(P<0.05).Before treatment,there was no difference in corneal center thickness,average cell area,hexagonal cell ratio,coefficient of variation of cell area,and corneal endothelial cell density between two groups(P>0.05).After treatment,corneal center thickness,average cell area,hexagonal cell ratio,and coefficient of variation of cell area in observation group were higher than control group,while corneal endothelial cell density in observation group was lower than control group,with significant difference(P<0.05).Effective rate of treatment in observation group was higher than control group,with no difference(P>0.05).Incidence of complications in observation group was lower than control group,with significant difference(P<0.05).Conclusion Phacoemulsification for cataract removal can achieve ideal curative effect for secondary glaucoma patients complicated with uveitis,can improve intraocular pressure,vision,and corneal endothelial cell condition significantly,with low incidence of complications,which is worthy of promotion and application.
