摘 要:目的评估3D图像融合技术应用于血管外科研究生血管腔内基本操作培训的效果。方法将2021年3月—2023年3月在中南大学湘雅二医院血管外科从事临床工作的研究生随机分为对照组和试验组,培训后分别在数字减影血管造影(DSA)引导下,以及在DSA+3D图像融合技术辅助下进行操作考核。比较两组的考核结果,包括操作成功率、操作次数、操作时长、射线时间、射线量及造影剂用量等指标。结果硕士研究生考核股动脉穿刺,对照组的操作次数高于试验组,两组比较差异有统计学意义(P<0.05);对照组的操作时长长于试验组,两组比较差异无统计学意义(P<0.05)。博士研究生考核分支动脉选择技术,对照组的操作时长、射线时间长于试验组,造影剂用量高于试验组,两组比较差异有统计学意义(P<0.05);对照组射线量高于试验组,两组比较差异无统计学意义(P>0.05)。结论应用3D图像融合技术能够显著提升血管外科研究生的血管腔内技术水平及操作熟练程度,减少操作过程中的射线剂量和造影剂用量,具有推广价值。Objective To evaluate effect of 3D image fusion technology in training of basic vascular operations for graduate students in vascular surgery.Methods The paper chose graduate students worked in Vascular Surgery Department of Xiangya Second Hospital of Central South University from March 2021 to March 2023,and divided them into control group and experimental group randomly.After training,students were assessed under DSA guidance and assisted by DSA+3D image fusion technology.Assessment results were compared between two groups,including indicators of operation success rate,operation frequency,operation duration,radiation time,radiation dose and contrast agent dosage.Results During assessment of femoral artery puncture for master students,number of operations in control group was higher than experimental group,difference between two groups was statistically significant.Operation time of control group was longer than experimental group,there was no statistically significant difference between two groups.During assessment of branch arteries selection of doctoral students,control group had longer operation time and radiation time than experimental group,and higher amount of contrast agent than experimental group,difference between two groups was statistically significant.Radiation dose in control group was higher than experimental group,there was no statistically significant difference between two groups.Conclusion 3D image fusion technology can improve intravascular technical level and operational proficiency of vascular surgery graduate students significantly,reduce radiation dose and contrast agent dosage during the operation process,which has promotional value.
