摘 要:目的探讨炎症因子、T淋巴细胞亚群水平与费城染色体(Ph)阴性骨髓增殖性肿瘤(MPN)患者预后的关系。方法选取50例Ph阴性MPN患者,均采取口服羟基脲片、皮下注射α干扰素治疗。随访6个月,根据预后情况分为预后良好组(n=23)和预后不良组(n=27)。比较两组患者的炎症因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]水平、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))水平。采用Spearman相关分析法分析炎症因子、T淋巴细胞亚群水平与Ph阴性MPN患者预后的相关性。结果预后不良组患者的IL-6、IL-8、TNF-α水平均高于预后良好组,差异均有统计学意义(P﹤0.05)。预后不良组患者的CD3^(+)、CD4^(+)水平均明显低于预后良好组,CD8^(+)水平明显高于预后良好组,差异均有统计学意义(P﹤0.01)。IL-6、IL-8、TNF-α、CD8^(+)水平与Ph阴性MPN患者预后不良均呈正相关(P﹤0.05)。CD3^(+)、CD4^(+)水平与Ph阴性MPN患者预后不良均呈负相关(P﹤0.01)。结论炎症因子、T淋巴细胞亚群水平与Ph阴性MPN患者的预后密切相关。Objective To explore the relationship between inflammatory cytokine,T lymphocyte subset levels and prognosis of patients with Philadelphia chromosom(Ph)-negative myeloproliferative neoplasm(MPN).Method A total of 50 Ph-negative MPN patients were selected,and all patients were treated with oral hydroxyurea tablets and subcutaneous injection of alpha interferon.Followed up for 6 months,the patients were divided into good prognosis group(n=23)and poor prognosis group(n=27)according to prognosis.The levels of inflammatory factors[interleukin-6(IL-6),interleukin8(IL-8),tumor necrosis factor-α(TNF-α)]and T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+))were compared between the two groups.Spearman correlation analysis was conducted to analyze the correlation between inflammatory factor,T lymphocyte subset levels and prognosis of Ph-negative MPN patients.Result The levels of IL-6,IL-8 and TNF-αin the poor prognosis group were higher than those in the good prognosis group,and the differences were statistically significant(P<0.05).The levels of CD3^(+)and CD4^(+)in the poor prognosis group were significantly lower than those in the good prog-nosis group,and the level of CD8^(+)was significantly higher than that in the good prognosis group,and the differences were statistically significant(P<0.01).The levels of IL-6,IL-8,TNF-αand CD8^(+)were positively correlated with poor prognosis of Ph-negative MPN patients(P<0.05).The levels of CD3^(+)and CD4^(+)were negatively correlated with poor prognosis of Ph-negative MPN patients(P<0.01).Conclusion The levels of inflammatory factors and T lymphocyte subsets were closely related to the prognosis of Ph-negative MPN patients.
