血管瘤、血管畸形及血管瘤病的ICD-10编码探讨

摘　　要：血管瘤、血管畸形及血管瘤病具有相似的临床特征,表现为暗红色或蓝紫色肿物突出于皮肤或者深部内脏器官,但其临床综合表现及组织形态学不同。血管瘤的病理特征是血管内皮细胞异常增生,被定义为肿瘤,其组织形态学为毛细血管瘤的应编码至(M9131/0)D18.0,卡波西样血管内皮瘤的应编码至(M9130/1)D48.1。血管畸形的组织结构没有内皮细胞的增殖,并非肿瘤,其组织形态学为毛细血管畸形的应编码至Q82.5,静脉畸形编码至Q27.9,动静脉畸形编码至Q27.3。血管瘤病则是有血管瘤相似临床表现但同时伴有其它特点的综合征或另类疾病,脑三叉神经血管瘤病编码至Q85.8,肝血管瘤病编码至K76.4。编码人员需要掌握这些疾病分类的知识,通过阅读病历中的细节,将其正确归类。Hemangioma,vascular malformation and hemangiomatosis have similar clinical features,which are dark red or blue-purple masses protruding from the skin or deep visceral organs,but their clinical manifestations and histopathological characteristics are different.The pathological characteristics of hemangioma are abnormal proliferation of vascular endothelial cells and are defined as tumors.The histopathological characteristics of capillary hemangioma should be coded to(M9131/0)D18.0,and Kaposiform hemangioendothelioma should be coded to(M9130/1)D48.1.Vascular malformations are not tumors,and their histological structures do not have endothelial cell proliferation.Their codes are:capillary malformations-Q82.5,venous malformations-Q27.9,arteriovenous malformations-Q27.3.Hemangiomatosis is a syndrome or other disease with hemangioma-like clinical manifestations but also with other characteristics.Trigeminal neuroangiomatosis is coded as Q85.8,and hepatic hemangiomatosis is coded as K76.4.Coding personnel need to master the knowledge of these disease classifications and classify them correctly by reading the details in the medical records.
