超声乳化联合小梁切除术治疗老年性白内障合并青光眼的临床效果分析

摘　　要：目的 探讨超声乳化联合小梁切除术治疗老年性白内障合并青光眼的临床效果分析。方法 将2021年1月-2023年12月在我院治疗的84例老年性白内障合并青光眼患者,根据手术方式分为两组:对照组单用小梁切除术,观察组使用超声乳化联合小梁切除术,对比两组的各项治疗指标、并发症、前房深度及眼轴长度。结果 观察组术后3个月的视力(0.70±0.15)高于对照组的(0.43±0.09),眼压(15.26±3.38) mmHg、视野缺损值(12.01±2.96) dB低于对照组的(22.94±4.69) mmHg、(14.75±3.11) dB (t=10.003,8.610,4.136,P<0.05),差异有统计学意义;观察组短暂性眼压升高、前房出血、角膜水肿、后囊膜破裂、虹膜炎等并发症发生率为4.76%,低于对照组的19.05%(χ^(2)=4.087,P<0.05),差异有统计学意义;观察组术后3个月的前房深度(2.43±0.31) mm大于对照组的(2.02±0.25) mm(t=6.672,P<0.05),差异有统计学意义。但两组眼轴长度(22.47±1.08) mm、(22.36±1.11) mm相当(t=0.460,P>0.05),两组间无统计学差异。结论 超声乳化联合小梁切除术治疗老年性白内障合并青光眼的临床效果显著,能有效提高视力,降低眼压及视野缺损,增加前房深度,减少并发症风险。Objective To explore the clinical efficacy analysis of phacoemulsification combined with trabeculectomy in the treatment of senile cataract with glaucoma.Methods Eighty-four patients with senile cataract and glaucoma treated in our hospital from January 2021 to December 2023 were divided into two groups according to the surgical methods:the control group underwent trabeculectomy alone,while the observation group underwent phacoemulsification combined with trabeculectomy.Various treatment indicators,complications,anterior chamber depth,and axial length were compared between the two groups.Results Three months postoperatively,the visual acuity of the observation group(0.70±0.15)was higher than that of the control group(0.43±0.09),intraocular pressure(15.26±3.38)mmHg,and visual field defect value(12.01±2.96)dB were lower than the control group(22.94±4.69)mmHg,(14.75±3.11)dB(t=10.003,8.610,4.136,P<0.05),with statistically significant differences;The incidence of complications such as transient intraocular pressure elevation,anterior chamber hemorrhage,corneal edema,posterior capsule rupture,and iritis in the observation group was 4.76%,which was lower than 19.05%in the control group(χ^(2)=4.087,P<0.05),with statistically significant differences;The anterior chamber depth of the observation group three months postoperatively(2.43±0.31)mm was greater than that of the control group(2.02±0.25)mm(t=6.672,P<0.05),with statistically significant differences.However,the axial lengths of the two groups(22.47±1.08)mm,(22.36±1.11)mm were comparable(t=0.460,P>0.05),with no statistical difference between the two groups.Conclusion Phacoemulsification combined with trabeculectomy shows significant clinical effect in the treatment of senile cataract complicated with glaucoma,which can effectively improve vision,reduce intraocular pressure and visual field defects,increase the depth of anterior chamber and reduce the risk of complications.
