摘 要:目的:探讨不同碘浓度造影剂在脂肪肝合并肝癌患者肝脏CT增强扫描中的应用价值。方法:于2021年8月至2023年3月,选择菏泽医学专科学校附属医院收治的脂肪肝合并肝癌患者78例,所有患者均在影像科接受肝脏CT增强扫描检查,根据造影剂碘浓度不同分为两组,A组(40例)碘浓度370 mg/ml,B组(38例)碘浓度300 mg/ml,每次造影剂总量为1.2 ml/kg,注射速度为3.5 ml/s,比较分析两组患者肝脏CT增强检查情况。结果:A组肝实质、主动脉、门静脉不同时期增强CT值均高于B组(P<0.05);A组图像质量主观评分优于B组(P<0.05);A组动脉期、门脉期及延迟期的肝癌病灶阳性检出率高于B组(P<0.05);A组与B组造影剂相关不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在脂肪肝合并肝癌患者肝脏CT增强扫描中应用高浓度(370 mg/ml)碘海醇可获得更高的增强值及图像质量,提升肝癌检出率,且不会明显增加造影剂相关不良反应。Objective:To explore the application value of different iodine concentration contrast agents in liver CT enhanced scanning of patients with fatty liver and liver cancer.Methods:From August 2021 to March 2023,78 patients with fatty liver and liver cancer admitted to our hospital were selected.All patients underwent liver CT enhanced scanning in the imaging department and were divided into two groups based on the different iodine concentrations of the contrast agent.Group A had 40 patients with iodine concentration of 370 mg/ml,and Group B had 38 patients with iodine concentration of 300 mg/ml.The total amount of contrast agent per injection was 1.2 ml/kg,and the injection rate was 3.5 ml/s.The liver CT enhanced examination results of the two groups of patients were compared and analyzed.Results:The enhanced CT values of liver parenchyma,aorta and portal vein in group A were higher than those in group B(P<0.05).The subjective score of image quality in group A was better than that in group B(P<0.05).The positive detection rate of liver cancer lesions in arterial phase,portal phase and delayed phase in group A was higher than that in group B(P<0.05).There was no significant difference in the incidence of contrast-related adverse reactions between group A and group B(P>0.05).Conclusion:The application of high concentration(370 mg/ml)iohexol in liver CT enhanced scan of patients with fatty liver and liver cancer can obtain higher enhancement value and image quality,improve the detection rate of liver cancer,and will not significantly increase the contrast agent-related adverse reactions.
