苯噻啶片联合琥珀酸舒马普坦片对偏头痛患者临床症状及脑血流动力学的影响  

The Effect of the Combination of Pizotifen Tablets and Sumatriptan Succinate Tablets on Clinical Symptoms and Cerebral Hemodynamics in Migraine Patients

摘　　要：目的为分析苯噻啶片联合琥珀酸舒马普坦片对偏头痛患者临床症状及脑血流动力学的影响。方法回顾性选取2022年7月至2023年12月商丘市中医院收治的偏头痛患者(82例)为研究对象,根据治疗方案不同将其分为对照组(n=41,采用琥珀酸舒马普坦片治疗)和联合组(n=41,采用苯噻啶片联合琥珀酸舒马普坦片治疗)。比较两组临床疗效,治疗前后临床症状[头痛发作持续时间、头痛发作频次、视觉模拟评分(VAS)],脑血流动力学指标[基底动脉(BA)、大脑前动脉(ACA)、双侧大脑中动脉(MCA)、双侧大脑中动脉(MCA)、大脑后动脉(PCA)的平均血流速度(VM)],血清细胞因子[白细胞介素-1β(IL-1β)、超敏C反应蛋白(hs-CRP)、溶血磷脂酸(LPA)、基质金属蛋白酶-9(MMP-9)]水平,不良反应发生率。结果联合组临床总有效率(92.68%)明显高于对照组(75.61%)(P<0.05)。治疗后,联合组头痛发作持续时间较对照组明显缩短,VAS评分、头痛发作频次较对照组明显降低(P<0.05);联合组BAVM、ACAVM、MCAVM、PCAVM降低幅度明显大于对照组(P<0.05);联合组血清IL-1β、hs-CRP、LPA、MMP-9水平较对照组低(P<0.05)。联合组不良反应总发生率(12.20%)与对照组(17.07%)相比,差异无统计学意义(P>0.05)。结论苯噻啶片联合琥珀酸舒马普坦片治疗偏头痛效果显著,可有效改善临床症状、降低脑血流速度、缓解炎症反应,同时可降低血清MMP-9、LPA水平,且安全可靠。Objective To explore the effects of the combination of pizotifen tablets and sumatriptan succinate tablets on clinical symptoms and cerebral hemodynamics in migraine patients.Methods A retrospective study was conducted on 82 migraine patients admitted to Shangqiu Traditional Chinese Medicine Hospital from July 2022 to December 2023.According to different treatment plans,they were divided into a control group(n=41,treated with sumatriptan succinate tablets)and a combined group(n=41,treated with pizotifen tablets combined with sumatriptan succinate tablets).The clinical efficacy of two groups was compared,as well as the levels of the clinical symptoms[duration of headache attacks,frequency of headache attacks,visual analogue scale(VAS)],cerebral hemodynamic indicators[mean blood flow velocity(VM)of the basal artery(BA),anterior cerebral artery(ACA)VM,bilateral middle cerebral artery(MCA)VM,posterior cerebral artery(PCA)VM],serum cytokines[interleukin-1β(IL-1β),hypersensitive C-reactive protein(hs-CRP),lysophosphatidic acid(LPA),and matrix metalloproteinase-9(MMP-9)]before and after treatment,and the incidence of adverse reactions was compared between the two groups.Results Compared with the total effective rate of the two groups,the combined group(92.68%)was significantly higher than that of the control group(75.61%)(P<0.05).After treatment,the duration of headache attack in the combined group was significantly shorter than that in the control group,and the VAS score and headache attack frequency in the combined group were significantly lower than those in the control group(P<0.05);BAVM,ACAVM,MCAVM,PCAVM in the combind group were significantly lower than those in the control group(P<0.05);the levels of serum IL-1β,hs-CRP,LPA and MMP-9 in the combined group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the combined group(12.20%)and the control group(17.07%)(P>0.05).Conclusion The effect of pizotifen tablets combined with
