局部亚低温联合尼莫地平及前列地尔治疗蛛网膜下腔出血后脑血管痉挛的临床效果

摘　　要：目的探讨蛛网膜下腔出血(SHA)后脑血管痉挛(CVS)患者采用局部亚低温联合尼莫地平及前列地尔治疗的效果。方法选择2021年4月—2023年5月济南市长清区人民医院收治的SHA后CVS患者60例为研究对象,按随机数字表法将其分为对照组及观察组,每组30例。对照组采用局部亚低温联合前列地尔治疗,观察组在对照组基础上采用尼莫地平治疗。比较两组患者的CVS发生情况、大脑动脉血流速度及临床疗效。结果观察组的CVS发生率为13.33%,低于对照组的50.00%,差异有统计学意义(P<0.05)。治疗后,观察组的大脑前动脉、中动脉及后动脉血流速度均慢于对照组,组间差异有统计学意义(P<0.05)。观察组的治疗总有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05)。结论局部亚低温联合尼莫地平及前列地尔治疗SHA后CVS的效果显著,有利于减少CVS发生情况,改善大脑动脉血流速度。Objective To investigate the effect of local mild hypothermia combined with nimodipine and alprostadil in the treatment of cerebral vasospasm(CVS)after subarachnoid hemorrhage(SHA).Methods Sixty patients with CVS after SHA admitted to Changqing District People's Hospital of Jinan City from April 2021 to May 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 30 cases in each group.The control group was treated with local mild hypothermia combined with alprostadil,and the observation group was treated with nimodipine based on the control group.The incidence of CVS,cerebral artery blood flow velocity and clinical efficacy of the two groups were compared.Results The incidence of CVS in the observation group was 13.33%,which was lower than 50.00%in the control group,and the difference was statistically significant(P<0.05).After treatment,the blood flow velocity of anterior cerebral artery,middle cerebral artery and posterior cerebral artery in the observation group were slower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of the observation group was 93.33%,which was higher than 73.33%of the control group,and the difference was statistically significant(P<0.05).Conclusion Local mild hypothermia combined with nimodipine and alprostadil in the treatment of CVS after SHA has a significant effect,which is conducive to reducing the occurrence of CVS and improving cerebral arterial blood flow velocity.
