涤痰化瘀汤对风痰入络型急性脑梗死患者中医证候及脑血流动力学的影响

摘　　要：目的分析涤痰化瘀汤在风痰入络型急性脑梗死(ACI)患者中的治疗效果。方法回顾性选取2021年10月—2022年10月临沂市中医医院收治的98例风痰入络型ACI患者的病历信息,按治疗方案的不同将其分为对照组和观察组,每组49例。对照组采用常规西医治疗,观察组在对照组基础上采用涤痰化瘀汤治疗,两组均连续治疗2周。对比两组患者的临床疗效、中医证候评分、脑血流动力学、日常生活能力及不良反应发生情况。结果观察组治疗总有效率为93.88%,高于对照组的79.59%,差异有统计学意义(P<0.05)。治疗后,两组各项中医证候评分均低于治疗前,且观察组均低于对照组,差异有统计学意义(P<0.05);两组基底动脉的阻力指数(RI)均低于治疗前,收缩期最大血流速度(Vs)、平均血流速度(Vm)均快于治疗前,日常生活活动能力量表(ADL)评分均高于治疗前,且观察组基底动脉的RI低于对照组,Vs、Vm均快于对照组,ADL评分高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论涤痰化瘀汤能够减轻风痰入络型ACI患者各项症状,调节脑血流动力学,有利于日常生活能力的提高,且不良反应较少。Objective To analyze the therapeutic effect of Ditan Huayu Decoction in patients with acute cerebral infarction(ACI)of wind-phlegm entering collaterals type.Methods The medical records of 98 patients with ACI of wind-phlegm entering collaterals type treated in Linyi Hospital of Traditional Chinese Medicine from October 2021 to October 2022 were retrospectively selected,and they were divided into a control group and an observation group according to different treatment plans,with 49 cases in each group.The control group was treated with conventional Western medicine,and the observation group was treated with Ditan Huayu Decoction on the basis of the control group.Both groups were treated continuously for 2 weeks.The clinical efficacy,TCM syndrome score,cerebral hemodynamics,ability of daily living and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.88%,which was higher than 79.59%of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of TCM syndrome in both groups were lower than those before treatment,and the observation group were lower than those in the control group,the difference was statistically significant(P<0.05);the basilar artery Resistance Index(RI)in both groups were lower than those before treatment,the maximum systolic blood flow(Vs)and average blood flow velocity(Vm)were faster than those before treatment,the Activies of Daily Living Scale(ADL)scores were higher than those before treatment,and the basilar artery RI in observation group was lower than that in the control group,Vs and Vm were faster than those in the control group,ADL scores was higher that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Ditan Huayu Decoction can reduce the symptoms of wind-phlegm entering collaterals type of ACI patients,regulate cerebral hemo
