单孔加一孔与传统多孔腹腔镜胃癌D2根治术在胃癌患者临床治疗中的应用价值对比

摘　　要：目的探讨单孔加一孔与传统多孔腹腔镜胃癌D2根治术在胃癌患者临床治疗中的应用价值对比。方法选取2021年1月—2023年10月滕州市中医医院收治的72例胃癌患者为研究对象,按随机数字表法将其分为对照组和观察组,每组36例。对照组采用传统多孔腹腔镜胃癌D2根治术治疗,观察组采用单孔加一孔腹腔镜胃癌D2根治术治疗。对比两组患者的手术情况、术后恢复情况、应激指标、生活质量。结果观察组手术时间为(315.44±32.58)min,长于对照组的(271.41±25.63)min,切口长度为(5.86±1.13)cm,短于对照组的(5.86±1.13)cm,术中出血量为(168.99±15.82)mL,少于对照组的(289.65±25.41)mL,组间差异有统计学意义(P<0.05);观察组首次进食、首次通气、首次排便及胃管拔除时间均短于对照组,组间差异有统计学意义(P<0.05);术后24 h,观察组去甲肾上腺素、皮质醇、促肾上腺皮质激素水平均低于对照组,组间差异有统计学意义(P<0.05);术后3个月,观察组世界卫生组织生活质量测定简表各个维度评分均高于对照组,组间差异有统计学意义(P<0.05)。结论单孔加一孔胃癌D2根治术治疗胃癌效果更佳,可减轻患者手术创伤,降低其应激水平,加快患者术后恢复,以提高其生活质量。Objective To explore the clinical value of single-port plus one-port and traditional multi-port laparoscopic radical D2 gastrectomy for gastric cancer in the treatment of gastric cancer.Methods Seventy-two patients with gastric cancer admitted to Tengzhou Hospital of Traditional Chinese Medicine from January 2021 to October 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 36 cases in each group.The control group was treated with traditional multi-port laparoscopic radical D2 gastrectomy for gastric cancer,and the observation group was treated with single-port plus one-port laparoscopic radical D2 gastrectomy for gastric cancer.The operation,postoperative recovery,stress indicators and quality of life of the two groups were compared.Results The operation time in the observation group was(315.44±32.58)min,which was longer than(271.41±25.63)min in the control group,the incision length was(5.86±1.13)cm,which was shorter than(5.86±1.13)cm in the control group,and the intraoperative blood loss was(168.99±15.82)mL,which was less than(289.65±25.41)mL in the control group,and the differences between the groups were statistically significant(P<0.05);the time of first feeding,first ventilation,first defecation and gastric tube removal in the observation group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05);24 hours after surgery,the levels of norepinephrine,cortisol and adrenocorticotropin in observation group were lower than those in control group,and the differences between the groups were statistically significant(P<0.05);3 months after surgery,the scores of various dimensions in the World Health Organization Quality of Life-Brief Version of the observation group were all higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Laparoscopic radical D2 gastrectomy for gast
