摘　　要：全球近一半人口存在幽门螺杆菌(Hp)感染,我国是Hp感染高发国家。慢性胃炎是我国人群最常见的消化系统疾病,Hp感染是其发生的主要病因。在患有慢性胃炎的基础上,部分Hp感染者可能会发展为胃黏膜萎缩和/或肠化生,进一步增加患胃癌的风险。胃黏膜屏障是机体隔绝Hp的第一道防线,包括黏液-碳酸氢盐屏障、上皮屏障、微循环屏障、生物屏障和免疫屏障,其结构与功能的破坏在Hp胃炎的发生发展中有着重要作用。益生菌可通过增强胃黏膜屏障,提高Hp根除率并减少治疗相关的副作用,从而在Hp胃炎的治疗中发挥重要作用。本文就Hp感染对胃黏膜屏障的影响及益生菌的干预效应进行深入综合分析,为Hp胃炎的防治提供科学依据。Nearly half of the global population is infected with helicobacter pylori(Hp),China is a country with a high incidence of Hp infection.Chronic gastritis is the most common digestive system disease in the Chinese population,Hp infection is the main cause of its occurrence,and some Hp infected individuals may develop gastric mucosal atrophy and/or intestinal metaplasia on the basis of chronic gastritis,further increasing the risk of gastric cancer.The gastric mucosal barrier is the body’s first line of defense against Hp,including the mucus bicarbonate barrier,epithelial barrier,microcirculation barrier,biological barrier,and immune barrier.The destruction of its structure and function plays an important role in the occurrence and development of Hp gastritis.Probiotics play an important role in the treatment of Hp gastritis by enhancing the gastric mucosal barrier,increasing the eradication rate of Hp,and reducing treatment-related side effects.This article provides a comprehensive analysis of the impact of Hp infection on the gastric mucosal barrier and the intervention effect of probiotics,providing a scientific basis for the prevention and treatment of Hp gastritis.
