祛腐生肌法对肛周脓肿及肛瘘患者术后创面愈合的效果研究

摘　　要：目的:分析祛腐生肌法对肛周脓肿及肛瘘患者术后创面愈合的效果。方法:选取2022年3月—2024年3月天津市东丽区中医医院收治的60例肛周脓肿及肛瘘术患者为研究对象,根据随机数字表法将其随机分为对照组和研究组,各30例。对照组采用康复新液纱条外敷治疗,研究组采用中医祛腐生肌法治疗。比较两组的创面相关指标、创面面积减情况、临床指标、疼痛指标。结果:研究组治疗有效率为96.67%,高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗后,研究组创面腐肉脱落、愈合和新生皮出现时间均短于对照组,研究组创面面积减小率为(67.52±20.45)%,高于对照组的(50.69±19.14)%,差异有统计学意义(P<0.05)。治疗后,研究组分泌物、肉芽形态和切口红肿评分、去甲肾上腺素水平和肛周疼痛数字评分法评分均低于对照组,差异均有统计学意义(P<0.05)。结论:祛腐生肌法治疗肛周脓肿及肛瘘术后创面,有助于缩短创面腐肉脱落时间、愈合时间和新生皮出现时间,降低创面面积减小率,改善患者临床指标,降低去甲肾上腺素水平,缓解肛周疼痛。Objective:To analyze the effect of removing saprophylaxis and promoting muscle method on patients with wound healing after operation of perianal abscess and anal fistula.Methods:A total of 60 patients with wounds after operation of perianal abscess and anal fistula treated in Dongli District Hospital of Traditional Chinese Medicine from March 2022 to March 2024 were selected as the research objects,and randomly divided into a control group and a study group according to random number table method,with 30 cases in each group.The control group was treated with Kangfu Xinye,and the study group was treated with removing saprophyte and promoting muscle method.The wond related indicatos,wound area reduction,clinical indicators,pain indexes were compared between the two groups.Results:The effective rate of the study group was 96.67%,higher than 80.00%of the control group,the difference was statistically significant(P<0.05).After treatment,the time of wound shedding,healing and new skin appearance in the study group were shorter than those in the control group,the reduction rate of wound area in the study group was(67.52±20.45)%higher than(50.69±19.14)%in the control group,the difference was statistically significant(P<0.05).After treatment,the scores of secretions,granulation morphology,incision reswelling,and norepinephrine level and perianal Numerical Pain Scale score in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The treatment of perianal abscess and anal fistula by removing saprophylaxis and promoting muscle method can help shorten the time of wound shedding,healing and appearance of new skin,reduce the reduction rate of wound area,improve clinical indicators,reduce norepinephrine level and relieve perianal pain.
