多层螺旋CT在胃癌患者术前分期诊断中的应用价值

摘　　要：目的:探究多层螺旋CT在胃癌患者术前分期诊断中的应用价值。方法:回顾性分析2019年1月—2023年12月咸宁市中心医院收治的50例胃癌手术患者资料,所有患者术前均进行多层螺旋CT诊断,以手术病理学为金标准,统计多层螺旋CT的诊断结果、诊断效能、灌注参数。结果:多层螺旋CT的T分期诊断准确度是94.00%、敏感度是90.00%、特异度是96.67%,N分期诊断准确度是98.00%、敏感度是95.00%、特异度是100.00%,M分期诊断准确度是100.00%、敏感度100.00%、特异度100.00%。胃癌病灶的血容量、渗透性表面容积均高于胃壁正常组织,M分期胃癌病灶的血容量、渗透性表面容积均高于N分期、T分期,差异均有统计学意义(P<0.05)。胃癌病灶、胃壁正常组织的血流量、通过时间比较,T分期、N分期、M分期胃癌病灶的血流量、通过时间比较,差异均无统计学意义(P>0.05)。结论:在胃癌患者术前分期诊断中应用多层螺旋CT,可通过灌注参数识别胃癌病灶组织和胃壁正常组织,进一步明确T、N、M分期。Objective:To explore the application value of multi-slice spiral CT in the diagnosis of preoperative staging of patients with gastric cancer.Methods:A total of 50 patients with gastric cancer undergoing surgery admitted to Xianning Central Hospital from January 2019 to December 2023 were selected as the study objects.All patients were diagnosed by multi-slice spiral CT before surgery.The diagnostic results,diagnostic efficiency and perfusion parameters of multi-slice spiral CT were analyzed using surgical pathology as the gold standard.Results:The diagnostic accuracy,sensitivity and specificity of multi-slice spiral CT in T stage were 94.00%,90.00%and 96.67%,the diagnostic accuracy,sensitivity and specificity of N stage were 98.00%,95.00%and 100.00%.The accuracy,sensitivity and specificity of M staging were 100.00%,100.00%and 100.00%respectively.The blood volume and permeable surface volume of gastric cancer lesions were higher than those of normal gastric wall tissue,the blood volume and permeable surface volume of gastric cancer lesions in M stage were higher than those in N stage and T stage,the differences were statistically significant(P<0.05).There were no significant differences in blood flow and passage time between gastric cancer lesions and normal gastric wall tissues,and between blood flow and passage time between gastric cancer lesions at T stage,N stage and M stage(P>0.05).Conclusion:In the preoperative staging diagnosis of patients with gastric cancer,multi-slice spiral CT can identify the lesion tissue and normal gastric wall tissue through perfusion parameters,and further determine the T,N and M staging.
